益气活血利水方治疗心衰病气虚血瘀水停证的临床研究

摘　　要：目的研究益气活血利水方治疗心衰病气虚血瘀水停证的临床疗效。方法选取2013年3月—2014年3月于北京中医药大学第二临床医学院心内科门诊和住院的80例心力衰竭病人,使用WPS表格生成随机数字表,按3∶1分成治疗组(60例)与对照组(20例)。对照组参照慢性心力衰竭指南规范用药,治疗组在对照组治疗基础上联合口服中药益气活血利水方,治疗4周后观测两组中医证候、主要症状积分、左室射血分数(LVEF)、N末端B型钠尿肽(NT-proBNP)、中西医结合生存质量量表评分等指标。结果治疗后,治疗组中医证候疗效优于对照组,其中乏力、心悸、胸闷(痛)等主要症状积分较对照组下降明显(P<0.05);治疗后,治疗组心功能分级、NT-proBNP较对照组下降明显(P<0.05);治疗组LVEF、中西医结合生存质量量表评分较对照组提高明显(P<0.05)。安全性方面,两组肝肾功能治疗前后比较,差异无统计学意义(P>0.05)。结论益气活血利水方可提升心衰病临床疗效,改善中医证候及症状评分,改善心功能,提升LVEF,降低NT-proBNP,提高病人生存质量。Objective To observe the effect of Yiqi Huoxue Lishui Recipe in the treatment of heart failure disease with Qi deficiency,Blood stasis and Fluid Retention syndrome.Methods From March 2013 to March 2014,80 patients were divided into the treatment group(n=60)and the control group(n=20)according to 3∶1.The control group was treated by the guidelines for chronic heart failure,and the treatment group was treated with Yiqi Huoxue Lishui Recipe on the basis of the control group.Four weeks later,traditional Chinese medicine syndromes,symptom scores,left ventricular ejection fraction(LVEF),N-terminal B-type natriuretic peptide(NT-proBNP),score of integrated Chinese and Western medicine quality of life scale were compared between two groups.Results The effective rate of traditional Chinese medicine syndrome in the treatment group was significantly higher than that in control group.Compared with the control group,fatigue,palpitations,and chest tightness in the treatment group were decreased significantly(P<0.05).Compared with the control group,New York Heart Association Heart Function Classification and NT-proBNP in the treatment group were decreasedsignificantly(P<0.05).LVEF and score of integrated Chinese and Western medicine quality of life scale in the treatment groups were increased significantly than that in the control group(P<0.05).There was no significant difference in safety between two groups(P>0.05).Conclusion Yiqi Huoxue Lishui Recipe could improve the clinical efficacy of heart failure,traditional Chinese medicine syndrome and symptoms score,improve heart function and LVEF,reduce NT-proBNP,improve the quality of life score of integrated traditional Chinese and western medicine.
