摘 要:目的探讨外周血单核细胞/淋巴细胞比值(MLR)对冠心病的诊断价值。方法回顾分析2018年3月~2019年5月我院302例接受冠状动脉造影(CAG)检查患者的临床资料,根据CAG结果分为冠心病组(220例)和非冠心病组(82例),收集患者的一般资料及临床实验室结果,根据血常规计算MLR,通过Gensini法对冠脉狭窄程度进行评估,分析MLR与冠心病狭窄程度相关性,绘制ROC曲线评估MLR预测冠心病诊断价值。结果冠心病组中男性、高血压病史所占比及单核细胞计数、淋巴细胞计数、MLR高于非冠心病组,差异有统计学意义(P<0.05);多因素Logistic回归分析显示,MLR是冠心病的独立危险因素[OR=3.94,95%CI(1.200~12.950),P=0.002];Spearman相关性分析显示,Gensini评分与MLR水平呈正相关(r=0.437,P<0.05);ROC曲线分析显示,MLR值为0.18时,MLR预测冠心病的敏感性为69.03%,特异度为64.81%[AUC=0.727,95%CI(0.683~0.771),P=0.005]。结论MLR是冠心病发生的独立危险因素,且与冠心病狭窄程度呈正相关,对诊断冠心病有一定的诊断价值。Objective To explore the diagnostic value of peripheral blood mononuclear cell/lymphocyte ratio(MLR)for coronary heart disease.Methods The clinical data of 302 patients undergoing coronary angiography(CAG)examination in our hospital from March 2018 to May 2019 were retrospectively analyzed.According to the CAG results,they were divided into coronary heart disease group(220 cases)and non-coronary heart disease group(82 cases),Collect patients'general data and clinical laboratory results,calculate MLR based on blood routine,evaluate the degree of coronary artery stenosis by Gensini method,analyze the correlation between MLR and coronary heart disease stenosis,draw ROC curve to evaluate the value of MLR in predicting coronary heart disease diagnosis.Results The proportion of males,hypertension history,monocyte count,lymphocyte count,and MLR in the coronary heart disease group were higher than those in the non-coronary heart disease group,the difference was statistically significant(P<0.05);multivariate Logistic regression analysis showed that MLR was Independent risk factors for coronary heart disease[OR=3.94,95CI(1.200-12.950),P=0.002];Spearman correlation analysis showed that Gensini score was positively correlated with MLR level(r=0.437,P<0.05);ROC curve analysis showed that when the MLR value was 0.18,the sensitivity of MLR to predict coronary heart disease was 69.03%and the specificity was 64.81%[AUC=0.727,95%CI(0.683~0.771),P=0.005].Conclusion MLR was an independent risk factor for coronary heart disease,and it was positively correlated with the degree of stenosis.MLR had certain diagnostic value for the diagnosis of coronary heart disease.
