摘　　要：慢性病在我国发病率逐年递增,不仅影响民众的健康和生命,还给家庭和社会带来沉重的经济负担。预防、控制和管理慢性病是卫生工作的重点。借鉴国内外慢性病管理经验和模式,广西医科大学第二附属医院针对慢性病防控中存在的问题,集中医院的技术和人才优势,充分利用医院平台,整合医疗资源,探索建立公立医院在慢性病管理中的作用和地位,以期为公立医院的慢性病管理提供依据和借鉴。The incidence of chronic disease is increasing year by year in China,affecting people′s health and life,and the family and society undertake a heavy economic burden.Prevention,control and management of chronic diseases is the focus of health work.Learning from the chronic disease management experience/model at home and abroad,the Second Affiliated Hospital of Guangxi Medical University aimed at the existing problems,concentrating hospital technology and talent advantages,making full use of the hospital platform,integrating the medical resources,and explored the role and status of public hospitals in chronic disease management,in order to provide the basis and reference for the chronic disease management of public hospitals.
