国医大师李济仁治疗痿病虚证用药剂量规律数据挖掘研究

摘　　要：目的利用数据挖掘技术分析国医大师李济仁治疗痿病虚证的用药剂量规律。方法收集李济仁治疗痿病虚证84例患者的195份病案临床有效处方,利用Medcase V3.2数据记录挖掘系统进行药物剂量值的数据挖掘与聚类分析。结果通过195份病案共得到159味药物及相应的414种剂量值,经筛选得到高频药物24味、高频剂量值51种,其中黄芪的单药频次最高(140),当归15 g的剂量值频次最高(86)。关联规则显示,当归15 g是药物关联的核心,与川芎15 g、穿山龙15 g、补骨脂20 g、千年健15 g等药物组成多组核心药对。通过K均值聚类分析并结合临床实际,形成四类具有药物剂量值的聚类方。结论李济仁治疗痿病虚证以固本培元法为主,以黄芪60 g与当归15 g为核心配伍加减补益药与活血通络药。Objective To discuss the dosage rules in treatment of atrophy disease by national TCM master Li Jiren based on data mining technology.Methods Totally 195 clinical effective prescriptions prescribed by Li Jiren for treatment 84 cases of atrophy disease with deficiency syndrome were collected for data mining and cluster analysis of drug dosage values by using Medcase V3.2 record mining system.Results In 195 prescriptions,a total of 159 kinds of medicine and corresponding 414 dosages were obtained.After screening,24 high-frequency medicines and 51 high-frequency dosages were obtained.Among them,Astragali Radix had the highest frequency of single medicine(140),and Angelicae Sinensis Radix had the highest frequency of dosage 15 g(86).Association rules showed that Angelicae Sinensis Radix 15 g was the core of medicine association,and it consisted of Chuanxiong Rhizoma 15 g,Dioscoreae Nipponicae Rhizoma 15 g,Psoraleae Fructus 20 g,and Homalomenae Rhizoma 15 g.Based on K-means clustering analysis and clinical practice,four types of clustering squares with medicine dosage values were formed.Conclusion Holding reinforcing the vital essence and strengthening the primordial qi as the principle,the treatment of atrophy disease with deficiency syndrome by Li Jiren uses modified strengthening body resistance and promoting blood circulation medicine with Astragali Radix 60 g and Angelicae Sinensis Radix 15 g as the core.
