瑞替普酶联合机械碎栓治疗亚急性期下肢深静脉血栓的疗效及安全性临床研究

摘　　要：目的研究瑞替普酶联合机械碎栓治疗亚急性期下肢深静脉血栓(LEDVT)的疗效及安全性。方法回顾性分析瑞替普酶联合机械碎栓治疗(n=74)与单用瑞替普酶的亚急性期LEDVT患者(n=63)的治疗,比较分析两组患者患肢消肿率、血栓清除率和PTS的发生率,评估两种治疗方案的疗效差异;比较分析两组患者的术后并发症发生情况来评估两种治疗方案的安全性。结果两种治疗方案均有效,通过对比两组患者的患肢大腿消肿率(P=0.40)、患肢小腿消肿率(P=0.92)、血栓清除率(P=0.19)发现近期疗效两种方案未见差异,通过对比两组患者PTS发生率发现联合治疗组在中期效果显著(P=0.01),两组患者术后常见并发症的发生率无显著差异(P=0.51),且均未发生严重不良事件。结论瑞替普酶联合机械碎栓治疗亚急性下肢深静脉血栓是一种安全有效的治疗方式,并且其发生PTS的概率更低。Objective To study the efficacy and safety of reteplase combined with mechanical thrombolysis in the treatment of subacute lower extremity deep vein thrombosis.Methods Retrospective analysis was performed on the treatment of patients with SUBacute LEDVT treated with reteplase combined with mechanical thrombotomy(n=74)and those treated with reteplase alone(n=63).Comparison was made between the two groups to evaluate the difference in efficacy between the two treatment regimens in terms of decongestive rate of the affected limb,thrombotic clearance rate and incidence of PTS.Postoperative complications were compared between the two groups to evaluate the safety of the two treatment regimens.Results Both treatments are effective,by comparing the two groups of patients limb leg swelling rate(P=0.40),legs limb swelling rate(P=0.92),blood clots clearance(P=0.19)found that in the near future curative effect two plans did not see difference,by comparing the two groups of patients the incidence of PTS combined treatment group(P=0.01)found in the medium term,the incidence of postoperative complications in both groups(P=0.51)has not been without obvious difference and serious adverse events.Conclusion Reteplase combined with mechanical thrombolysis is a safe and effective treatment for subacute deep vein thrombosis of lower extremity,and the incidence of PTS is lower.
