补肺汤合二陈汤加减联合盐酸氨溴索雾化吸入治疗痰热壅肺型急性支气管炎疗效及对肺通气功能、气道炎症指标的影响

摘　　要：目的:观察用补肺汤合二陈汤加减联合盐酸氨溴索雾化吸入治疗痰热壅肺型急性支气管炎疗效及对肺通气功能、气道炎症指标的影响。方法:选取124例急性支气管炎患者,随机分为对照组和观察组各62例,对照组患者给予盐酸氨溴索雾化吸入治疗,观察组患者在此基础上加用补肺汤合二陈汤加减治疗,连续用药7 d。对比两组的临床疗效、呼吸道症状改善时间,比较肺通气功能,监测气道炎症指标变化。结果:观察组临床有效率较对照组明显升高(P<0.05);观察组患者发热消失时间,咳痰、咳嗽症状的改善时间明显短于对照组(P<0.05);肺通气功能指标用力肺活量(Forced Vital Capacity,FVC)、第一秒用力呼气量(Forced Expiratory Volume in 1 Second,FEV1)、FEV1/FVC、呼气峰流速(Peak Expiratory Flow,PEF)、动态肺顺应性(Dynamic Lung Compliance,Cdyn)较对照组显著升高,呼吸指数(Respiratory Index,RI)水平则较对照组明显降低(P<0.05);患者治疗后的气道炎症指标[降钙素原(Procalcitonin,PCT)、超敏C反应蛋白(High-sensitivity C-reactive protein,hs-CRP)、白细胞介素-8(Interleukin-8,IL-8)]水平较对照组明显降低(P<0.05)。结论:补肺汤合二陈汤加减联合盐酸氨溴索雾化吸入利于促进痰热壅肺型急性支气管炎患者呼吸道症状的缓解,提高肺功能,减轻气道炎症,值得临床推广运用。Objective:To observe the effect of the Bufei decoction(补肺汤)and the Erchen decoction(二陈汤)with ambroxol hydrochloride nebulized inhalation on acute bronchitis of the Tanre Yongfei type(痰热壅肺型),lung ventilation function and airway inflammation indicators.Methods:124 patients with acute bronchitis were randomly divided into the control group and the observation group,with 62 cases in each.The control group was given ambroxol hydrochloride nebulized inhalation,and the observation group was treated with the Bufei decoction and the Erchen decoction more,continuous medication for 7 days.Results:The clinical effective rate in the observation group was significantly higher than the control group(P<0.05).The time of disappearance of fever,sputum and cough symptoms in the observation group were significantly shorter than the control group(P<0.05).The lung ventilation function indexes,such as FVC,FEV1,FEV1/FVC,PEF,Cdyn,were significantly higher than those in the control group.The RI level was significantly lower than that in the control group(P<0.05).The levels of PCT,hs-CRP,IL-8 were significantly lower than those in the control group(P<0.05).Conclusion:The Bufei decoction and the Erchen decoction with ambroxol hydrochloride nebulized inhalation is beneficial to promote the alleviation of respiratory symptoms in patients with acute bronchitis of the Tanre Yongfei type,improve lung function,and reduce airway inflammation.It is worthy of clinical application.
