血清MCP-1、omentin联合ES检测诊断多囊卵巢综合征的临床意义

摘　　要：目的:探究检测血清中单核细胞趋化蛋白-1(MCP-1)、血清网膜素(omentin)、内皮抑素(ES)水平多诊断囊卵巢综合征临床价值。方法:选取2018年3月-2019年3月本院收治的多囊卵巢综合征患者96例(多囊卵巢组),68例库欣综合征患者(库欣组),体检健康志愿者36例(对照组)。比较各组血清MCP-1、omentin、ES水平。分析单独及联合检测3项指标评估多囊卵巢综合征效果。采用ROC曲线分析各评估方法效率。结果:多囊卵巢组血清MCP-1、omentin水平低于库欣组和对照组, ES水平高于库欣组和对照组,而库欣组MCP-1及omentin水平均低于对照组、ES水平高于对照组(均P<0.05);MCP-1+omentin+ES系列联合检测的敏感度低于MCP-1+omentin+ES平行联合检测,特异度高于平行联合检测(P<0.05);MCP-1+omentin+ES平行联合检测评估多囊卵巢综合征ROC曲线下面积(0.902)高于系列联合检测(0.889)(P>0.05)。结论:MCP-1+omentin+ES联合评估能有效提高多囊卵巢综合征病情评估敏感性、特异性及准确率。Objective: To explore the value of the levels of serum monocyte chemoattractant protein-1(MCP-1), omentin, and endostatin(ES) level for diagnosing polycystic ovary syndrome(PCOS). Methods: 96 women with PCOS were selected in group A, 68 women with Cushing syndrome were selected in group B, and 36 healthy volunteers were selected in group C from March 2018 to March 2019. The serum levels of MCP-1, omentin, and ES of women were compared among the three groups. The value of the serum MCP-1 level, omentin level, or ES level only, and the value of combined the serum MCP-1 level, omentin level, and ES level for diagnosing PCOS were analyzed. The ROC curve was used to analyze the evaluation efficiency. Results: The serum levels of MCP-1 and omentin of women in group A were significant lower than those of women in group B and C, but the serum ES level of women in group A were significant higher(P<0.05). The serum levels of MCP-1 and omentin of women in group B were significant lower than those of women in group C, but the serum level of ES of women in group B were significant higher(P<0.05). The sensitivity of combined McP-1 level, omentin level, and ES level for diagnosing PCOS was significant lower than that of McP-1 level, omentin level, or ES level only, but the specificity of the combined of McP-1 level, omentin level, and ES level for diagnosing PCOS was significant higher(P<0.05). The area under ROC curve of McP-1 level, omentin level, or ES level only for diagnosing PCOS was 0.902, and that of the combined of McP-1 level, omentin level, and ES level for diagnosing PCOS was 0.889(P>0.05). Conclusion: The combined of McP-1 level, omentin level, and ES level for diagnosing PCOS can improve the sensitivity, the specificity, and the accuracy effectively.
