38例负压辅助愈合系统治疗心脏开胸手术难愈性切口的效果

摘　　要：目的探讨负压辅助治疗系统(Vacuum-assisted closure,VAC)治疗心外科难愈性切口的临床疗效。方法选取河北医科大学第二医院心脏外科2015年2月至2018年8月期间行心外科手术切口愈合不良的患者38例,按不同治疗方法分为VAC治疗组(18例)和普通换药组(20例),比较两组患者全身情况、切口感染、创面大小、创面细菌培养、实验室检查结果、疼痛评分、抗生素应用、住院花费情况。结果两组患者全身情况、白细胞、血红蛋白及白蛋白比较差异均无统计学意义(P均>0.05),VAC治疗组换药次数、抗生素使用时间、治疗时间均少于普通换药组(P均<0.01),两组患者治疗费用差异无统计学意义(P>0.05),VAC治疗组疼痛视觉评分低于普通换药组(P<0.05)。并且VAC治疗组患者治疗2~4次,可使创面新生大量新鲜肉芽组织。结论VAC治疗比传统换药更能有效促进心外科难愈性切口的愈合。Objective To investigate the effectiveness of vacuum-assisted closure(VAC)therapy in cardiac surgery on refractory incision.Mehtods A total of 38 patients with poor wound healing who underwent cardiac surgery from February 2015 to August 2018 in Hebei Medical University were divided into VAC group(n=18)and General Dressing Change group(n=21),the general condition,wound infection,wound size,wound bacterial culture,results of laboratory examination,pain score,antibiotic application and hospitalization expenses of the two groups were compared.Results There was no significant difference between the two groups in terms of body condition,white blood cell,hemoglobin and albumin(P all>0.05).The Times of dressing change,time of using antibiotics and time of treatment in Vac group were less than those in General Dressing Change group(P all<0.01),there was no significant difference in treatment cost between the two groups(P>0.05).The visual pain score of Vac group was lower than that of General Dressing Change group(P<0.05).Vac treatment group patients treated 2~4 times,the wound could produce a large number of fresh granulation tissue.Conclusion For refractory wounds in cardiac surgery,VAC treatment compared with general treatment,cardiac surgery in refractory wounds VAC therapy has obvious advantages.
