锥面螺旋髋臼假体在全髋关节置换术中的应用效果观察

摘　　要：目的探究双锥面螺旋髋臼假体在全髋关节置换术中的临床效果。方法选取本院2019年1月-2020年1月收治的59例初次行全髋关节置换术后假体松动的患者,术前均行X线、CT等影像学检查,术中彻底清理髋臼并根据骨缺损情况行同种异体颗粒松质骨打压植骨,随机分为对照组和实验组,对照组(29例)使用单锥形螺旋髋臼假体,实验组(30例)使用双锥面螺旋髋臼假体,术后进行随访,比较两组患者康复情况。结果两组患者手术时间、切口长度、术中失血量及住院总时间比较差异均无统计学意义(P均>0.05);两组患者术前Harris评分差异无统计学意义,但是对照组术后各个时间点得分均低于实验组(P均<0.05)。实验组的假体脱位发生率低于对照组(P<0.05),并且不良反应总发生率(6.7%)也低于对照组(20.0%)(P<0.05)。结论运用双锥面螺旋髋臼假体对初次行全髋关节置换术后松动的患者翻修治疗具有更安全、高效的优势。Objective To explore the clinical effectiveness of biconical screw acetabular prosthesis in total hip arthroplasty.Methods From January 2019 to January 2020,59 patients with loose prosthesis after total hip replacement for the first time were selected.All patients underwent X-ray,CT and other imaging examinations before operation,cleaned the acetabulum thoroughly during operation,and performed allogeneic granular cancellous bone compression and bone grafting according to bone defects.They were randomly divided into control group(29 cases)and experimental group(30 cases).The patients in the control group were treated with single conical spiral acetabular prosthesis,and the patients in the experimental group were treated with double conical spiral acetabular prothesis.Both were followed up after operation.Results The preoperative results showed no significant difference between the two groups of patients(P all>0.05),and when comparing the postoperative time points,the control group scored significantly lower than the experimental group(P all<0.05).The incidence of prosthesis dislocation was significantly lower in the experimental group(0)than in the control group(13.79%)(P<0.05),w hile the total incidence of adverse reactions(6.7%)was significantly lower than in the control group(20.00%),with a significant difference(P<0.05)between the two groups.Conclusion The use of a biconical spiral acetabular prosthesis for revision therapy in patients with loosening after initial total hip arthroplasty is safe and effective,with a higher Harris score for hip function and a lower incidence of prosthesis dislocation,resulting in better recovery of joint function and a higher safety profile.
