基于指南的血管可视化技术循证实践在儿童外周静脉留置针穿刺中的效果研究

摘　　要：目的观察基于指南的儿童外周静脉留置针穿刺血管可视化技术循证实践效果。方法将静脉输液治疗标准指南中得到的相关最佳证据应用到儿科病房外周静脉留置针穿刺血管可视化技术的护理实践中,以渥太华证据转化模式为指导框架,通过研究场所的现场评估、团队讨论构建循证策略和实践方案、循证方案临床实践以及对实践效果的系统审核和过程反思4个阶段的临床实践,比较循证护理实践前后护士对循证实践方案中各临床审查指标的依从性,护士进行留置针静脉穿刺一次成功率和穿刺时间以及患儿家长对血管可视化技术使用的满意程度。结果循证实践后护士对各审查指标的依从性显著提高(P<0.05);护士静脉留置一次穿刺成功率由82.98%提高到91.91%(P<0.05),留置针穿刺时间明显缩短(P<0.01);患儿家长对留置针穿刺的满意度提高。结论基于指南的儿童外周静脉留置针穿刺血管可视化技术循证实践可提高儿童静脉留置一次性穿刺成功率,缩短穿刺时间,提高家长满意度。Objective To study the effect of evidence-based practice on vascular visualization technology based on guidelines on pediatric peripheral venous indwelling needle puncture.Method Apply the best related evidence from standard guidelines of infusion on nursing practice of vascular visualization technology for pediatric peripheral venous indwelling needle puncture.Compare nurses’compliance on each clinical review index of evidence-based practical scheme,nurses’one-time success rate of puncture and puncture time and parents’satisfaction degree on application of vascular visualization technology before and after practice through 4 clinical practice stages of site assessment,constructing evidence-based strategy and practical scheme by group discussion,clinical application of evidence-based scheme,evaluation on practical effect by system and process reflection based on Ottawa Model of evidence transformation.Result Nurses’compliance on each evaluation index increases obviously after evidence-based practice(P<0.05).Nurses’one-time success rate of puncture improves from 82.98%to 91.91%(P<0.05).Puncture time is obviously shortened(P<0.01).Parents’satisfaction degree on indwelling needle puncture is obviously improved.Conclusion Evidence-based practice of vascular visualization technology based on guidelines on pediatric peripheral venous indwelling needle puncture can improve one-time success rate of puncture,shorten puncture time and improve parents’satisfaction degree.
