高频振荡通气配合多巴酚丁胺对新生儿PPHN患儿血清CRP、BNP、D-D水平的影响

摘　　要：目的:探讨高频振荡通气配合多巴酚丁胺对新生儿持续肺动脉高压(PPHN)患儿血清C反应蛋白(CRP)、B型钠尿肽(BNP)、D-二聚体(D-D)水平的影响。方法:研究对象选自我院于2016年2月—2018年12月收治的76例新生儿持续肺动脉高压患儿,随机分为2组,各38例。两组均给予常规治疗,对照组予以盐酸多巴酚丁胺注射液进行治疗,观察组加用呼吸机进行高频振荡通气治疗,两组均连续治疗2天。观察两组患儿血清CRP、BNP、D-D水平及血气指标水平。结果:治疗2天后,观察组血清CRP、BNP、D-D及PaCO2水平均低于对照组,观察组患儿PaO2、SaO2水平均高于对照组,差异有统计学意义(P<0.05)。结论:高频振荡通气配合多巴酚丁胺可有效改善新生儿PPHN患儿血清CRP、BNP、D-D水平及血气指标水平,有效缓解患儿肺动脉高压症状,疗效显著。Objective:To investigate the effects of high-frequency oscillatory ventilation combined with dobutamine on serum C-reactive protein(CRP),B-type natriuretic peptide(BNP)and D-dimer(DD)in children with persistent pulmonary hypertension(PPHN)Level of influence.Methods:The study subjects were selected from 76 neonates with persistent pulmonary hypertension admitted to our hospital from February 2016 to December 2018,and they were randomly divided into 2 groups with 38 cases in each group.Both groups were given conventional treatment.The control group was treated with dobutamine hydrochloride injection,and the observation group was treated with a ventilator for high-frequency oscillatory ventilation.Both groups were treated for 2 consecutive days.Observe the serum CRP,BNP,D-D levels and blood gas index levels in the two groups.Results:After 2 days of treatment,the serum CRP,BNP,D-D and PaCO2 levels of the observation group were lower than those of the control group,and the PaO2 and SaO2 levels of the children in the observation group were higher than those of the control group.The difference was statistically significant(P<0.05).Conclusion:High-frequency oscillatory ventilation combined with dobutamine can effectively improve the serum CRP,BNP,D-D levels and blood gas index levels in neonates with PPHN,and effectively relieve the symptoms of pulmonary hypertension in children with significant curative effects.
