摘  要:目的探讨特异性免疫治疗对支气管哮喘患儿血清调节性T淋巴细胞、IL-10、sIgE水平的影响,分析特异性免疫治疗对儿童哮喘的疗效。方法选取2017年2月至2018年4月贵州省贵阳市妇幼保健院儿童呼吸哮喘专科收治的支气管哮喘患儿60例,随机分为实验组和对照组,各30例。对照组患儿接受常规糖皮质激素吸入剂等基础用药治疗;实验组在基础用药治疗的同时给予特异性免疫(Specific immunotherapy,SIT)治疗。检测并对比CD4^+CD25^+T细胞、CD4^+CD25^high CD127^low Treg细胞占比变化,血清IL-10、sIgE水平变化。结果治疗前两组患儿咳喘发作次数与持续时间比较无显著差异(P>0.05);两组经过治疗1年后,患儿临床症状均明显改善;但实验组患儿平均咳喘次数、持续时间均明显低于对照组(P<0.05);治疗1年后两组CD4^+CD25^+T细胞、CD4^+CD25^high CD127^low Treg细胞、IL-10水平升高(P<0.05),实验组均高于对照组(P<0.05);1年后两组sIgE水平均下降,实验组均低于对照组(P<0.05)。结论对支气管哮喘患儿给予特异性免疫治疗可改善调节性T淋巴细胞水平,还可改善炎症因子、免疫指标水平,可改善哮喘临床症状,降低其复发率,有效缓解哮喘病情,随访观察一年疗效理想,值得推广。Objective To explore the effect of specific immunotherapy on the levels of serum regulatory T lymphocyte,IL-10 and sIgE in children with bronchial asthma,and to analyze its therapeutic effect.Methods According to the inclusion and exclusion criteria set out in the 2016 edition of the guidelines for the diagnosis and prevention of bronchial asthma in children,From February 2017 to April 2018,60 children with bronchial asthma were selected as the research objects who was from Guiyang maternal and child health hospital children's respiratory asthma specialty.Sixty children with bronchial asthma were selected and randomly divided into experimental group and control group with 30 cases each.The control group was given basic medication.The experimental group was given specific immunotherapy at the same time of basic medication treatment.The percentage of CD4^+CD25^+T cells,CD4^+CD25^high CD127^low Treg cells,the level of serum IL-10 and sIgE were detected and compared.Results There were no statistical significantly differences in the frequency and duration of cough and asthma between the two groups before treatment(P>0.05).After one year of treatment,the clinical symptoms of the two groups were significantly improved.The average times and duration of cough and asthma in the experimental group were significantly lower than those in the control group(P<0.05).One year after treatment,the levels of CD4^+CD25^+T cells,CD4^+CD25^high CD127^low Treg cells and IL-10 in the two groups increased(P<0.05),the experimental group was higher than the control group(P<0.05).After one year,the levels of SiGe decreased in both groups.The experimental group was lower than the control group(P<0.05).Conclusion Specific immunotherapy can improve the level of regulatory T lymphocytes in children with asthma.It can also improve the level of inflammatory factors and immune indexes,specific immunotherapy can improve the clinical symptoms of asthma,reduce its recurrence rate,and effectively alleviate the condition of asthma.The follow-up observat
