摘 要:目的观察活血补肾壮骨方对糖皮质激素性骨质疏松(GIOP)型大鼠骨折愈合的作用及对骨密度、血清碱性磷酸酶和骨钙素水平的影响。方法将60只SD大鼠随机分为空白组、模型组、西药组,中药(活血补肾壮骨方)低剂量组、中剂量组和高剂量组。除空白组外,其余各组肌内注射地塞米松(1 mg/kg)建模,每日1次,每周3次,连续9周,再行股骨骨折建模,建模后空白组、模型组给予生理盐水灌胃,西药组给予阿仑膦酸钠(5 mg/kg)灌胃,中药组给予活血补肾壮骨方灌胃,低剂量组2.5 mg/kg、中剂量组5 mg/kg、高剂量组7.5 mg/kg,每日1次,连续10周。分别测定各组大鼠的骨转换指标、骨密度、骨组织形态计量学指标、骨生物力学指标。结果与空白组比较,模型组抗酒右酸酸性磷酸酶(TrACP)、Ⅰ型胶原交联羧基端肽(CTX-1)、骨小梁分离度(Tb.Sp)明显升高,碱性磷酸酶(ALP)、骨钙素(OC)、骨密度(BMD)、组织骨密度(TMD)、骨小梁面积百分数(TbAr%)、骨小梁厚度(Tb.Th)、骨小梁数量(Tb.N)、最大载荷、最大挠度、最大应力明显降低(P<0.05);与模型组相比,各治疗组大鼠TrACP、CTX-1、Tb.Sp均明显降低(P<0.05),ALP、OC、BMD、TMD、TbAr%、Tb.N、Tb.Th、最大载荷、最大挠度、最大应力均明显升高(P<0.05);与西药组比较,高剂量组TrACP、CTX-1、Tb.Sp均降低明显(P<0.05),ALP、OC、BMD、TMD、TbAr%、Tb.N、Tb.Th、最大载荷、最大挠度、最大应力均升高明显(P<0.05)。结论活血补肾壮骨方能显著升高GIOP大鼠的血清ALP、OC含量,降低TrACP、CTX-I水平,增加BMD、Tb.N,缩小骨小梁间隙,改善骨显微结构,增强骨强度,提升骨生物力学性能,从而有效改善GIOP症状,促进骨折愈合,保护骨组织。Objective:To investigate the role of Huoxue Bushen Zhuanggu Recipe on fracture healing of glucocorticoid-induced osteoporosis(GIOP)rats and its effect on bone mineral density,serum alkaline phosphatase and osteocalcin levels.Methods:A total of 60 SD rats were randomly divided into the blank group,model group,western medicine group,Chinese medicine(Huoxue Bushen Zhuanggu Recipe)low-dose group,middle-dose group and high-dose group.Except for the blank group,the other groups were modeled by intramuscular injection of dexamethasone(1 mg/kg),once a day,three times per week for 9 consecutive weeks,and femoral fractures were modeled again.During the period,the blank group and the model group were given intragastric saline,the western medicine group was given alendronate sodium(5 mg/kg),the Chinese medicine group was given Huoxue Bushen Zhuanggu Recipe,with the low-dose group 2.5 mg/kg,medium-dose group 5 mg/kg and the high-dose group 7.5 mg/kg,once a day for 10 consecutive weeks.The bone turnover index,bone density,bone histomorphometry and bone biomechanics of rats in each group were measured.Results:Compared with the blank group,TrACP,CTX-1,Tb.Sp of the model group were significantly increased,ALP,OC,BMD,TMD,TbAr%,Tb.N,Tb.Th,maximum load,maximum deflection,and maximum stress were significantly reduced(P<0.05);compared with the model group,the TrACP,CTX-1,Tb.Sp in each treatment group decreased significantly(P<0.05),ALP,OC,BMD,TMD,TbAr%,Tb.N,Tb.Th,maximum load,maximum deflection,and maximum stress were significantly increased(P<0.05);compared with western medicine group,TrACP,CTX-1,Tb.Sp in high dose group decreased significantly(P<0.05),ALP,OC,BMD,TMD,TbAr%,Tb.N,Tb.Th,maximum load,maximum deflection,and maximum stress all increased significantly(P<0.05).Conclusion:Huoxue Bushen Zhuanggu Recipe can significantly increase the serum ALP and OC levels of GIOP rats,reduce the levels of TrACP and CTX-I,increase the bone density and the number of trabecular bones,reduce the trabecular bone space,improve the bone microstructu
