摘 要:目的:探讨数字骨科技术辅助个性化手术治疗骨盆髋臼骨折的应用价值。方法:回顾性分析2016年1月-2019年10月手术治疗的52例骨盆髋臼骨折患者。术前行骨盆薄层CT扫描获得原始数据,行三维重建并3D打印出1∶1骨盆模型,在模型上进行术前规划、模拟手术并预塑形钢板,术中根据模型指导手术。术后以Matta标准评估骨折复位情况,改良的Merle D’Aubigne和Postel评分系统评估髋关节功能情况。结果:所有患者均顺利完成手术,手术时间85~255min,术中出血量350~1600ml。随访0.5~2.5年,全部骨性愈合。术后1周骨折复位质量评估:优36例,良12例,可4例,优良率92.3%;术后6个月髋关节功能评估:优23例,良20例,可8例,差1例,优良率82.7%。结论:数字骨科技术辅助手术治疗骨盆髋臼骨折,可以优化手术方案,提高手术安全性和精准性,疗效满意,是实施个性化精准治疗理想的方法。Objective:To investigate application value of digital orthopedic techniques assisted personalized surgical treatment for pelvic or acetabular fractures.Methods:A retro-spective analysis was made on 52 patients with pelvic or acetabular fractures which treated with operation from January 2016 to October 2019.The pelvic lamella CT scan was performed before operation for obtaining the original data,then 3 D reconstruction was performed,and 1∶1 pelvis models were printed out.Preoperative planning,simulated surgery and pre-contouring plates were performed on the models,and operation was guided by the models intraoperative.Postoperative conditions of fracture reduction and hip joint function were evaluated by X-ray Matta evaluation criteria and modified Merle D’Aubigne and Postel scoring system.Results:All patients were completed the operation successfully.The operation time was 85~255 minutes,the amount of bleeding was 350~1600 ml.Follow-up 0.5~2.5 years,bone healing could be seen in all patients.The excellent and good rate of X-ray Matta score in 1 week postoperative was 92.3%,and there were 36 patients with excellent,12 patients with good and 4 with common.The excellent and good rate of hip joint function according to modified Merle D’Aubigne and Postel criteriain in postoperative 6 months was 82.7%,there were 23 patients with excellent,20 patients with good,8 patients with common and 1 patient with poor.Conclusion:Digital orthopedic techniques assisted personalized surgical treatment for pelvic or acetabular fractures may optimize operation program,improve the safety and precision of operation,and have good effects.It is an ideal personalized therapeutic method.
