摘 要:目的研究2型糖尿病患者粪便中6种短链脂肪酸水平与糖化血红蛋白的相关性。方法采用气相色谱法检测粪便中短链脂肪酸的含量,并对方法学进行考察。选取2018年在本院查体的2型糖尿病患者57例,根据糖化血红蛋白水平将其分为高糖化组(糖化血红蛋白>7.0,28例)与低糖化组(糖化血红蛋白≤7.0,29例)。应用气相色谱法检测两组患者粪便中6种短链脂肪酸水平,并分析6种短链脂肪酸水平与糖化血红蛋白的相关性。结果低糖化组患者粪便中乙酸、丙酸、丁酸、戊酸水平显著高于高糖化组(均P<0.05),异丁酸与异戊酸水平差异无统计学意义(均P>0.05)。Pearson相关性分析结果表明:在2型糖尿病患者中,糖化血红蛋白与粪便中乙酸呈负相关(r=-0.5401,P<0.0001),与丙酸呈负相关(r=-0.3581,P=0.0062),与丁酸呈负相关(r=-0.4217,P=0.0011),与戊酸呈负相关(r=-0.3268,P=0.0423),与异丁酸、异戊酸无相关性。结论2型糖尿病患者粪便中短链脂肪酸水平与糖化血红蛋白存在一定的相关性,短链脂肪酸水平的下降可能是影响血糖控制不佳的主要原因。Objective To study the correlation between the levels of six short chain fatty acids(SCFAs)in feces and glycosylated hemoglobin of patients with type 2 diabetes mellitus(T2 DM).Methods Gas chromatographic method for determination of SCFAs in feces was established and the methodology was investigated.57 patients with T2 DM who were examined in 2018 were selected and divided into the high(HbA1 c)group(HbA1 c>7.0,28 cases)or low HbA1 c group(HbA1 c≤7.0,29 cases)according to the HbA1 c level.Six kinds of SCFAs in feces in the two groups were determined using gas chromatography,and the correlation between the levels of the six SCFAs and HbA1 c was analyzed.Results The levels of acetic acid,propionic acid,butyric acid and valeric acid in the low HbA1 c group were significantly higher than those in the high HbA1 c group(all P<0.05);there were no significant differences in the levels of isobutyric acid and isovaleric acid(all P>0.05).Pearson correlation analysis showed that in the feces of patients with T2 DM,HbA1 c was negatively correlated with acetic acid(r=-0.5401,P<0.0001),propionic acid(r=-0.3581,P=0.0062),butyric acid(r=-0.4217,P=0.0011),and valeric acid(r=-0.3268,P=0.0423),but not with isobutyric acid and isovaleric acid.Conclusion There is a certain correlation between the levels of SCFAs in the feces and glycosylated hemoglobin of patients with type 2 diabetes;decreased levels of SCFAs may be an important cause of poor glycemic control.
