摘 要:目的:探讨自动乳腺超声成像(ABUS)及常规二维超声(US)检查中乳腺良性和恶性肿瘤的超声表现特点,旨在提高对乳腺良性与恶性肿瘤的超声鉴别诊断能力,为临床提供准确的参考信息。方法:回顾性收集本院2018年8月—2019年10月经临床病理证实的乳腺肿瘤患者50例,全部患者在术前均行ABUS及US检查,分析乳腺肿瘤的ABUS及US影像特征,并与临床手术后的病理结果进行对照,比较ABUS及US诊断乳腺良恶性肿瘤的准确度、灵敏度和特异度。结果:在50例乳腺肿瘤患者中,乳腺良性肿瘤28例,包括纤维腺瘤18例,积乳囊肿3例,腺病2例,良性叶状肿瘤5例;乳腺恶性肿瘤22例,包括浸润性导管癌12例,浸润性小叶癌5例,导管原位癌3例,恶性叶状肿瘤2例。ABUS检查结果:在冠状面重建图像上可见"汇聚征"的有15例乳腺肿瘤,12例病理证实为恶性,3例病理结果为良性,ABUS诊断乳腺良恶性肿瘤的准确度为89.23%,灵敏度为87.61%,特异度为91.54%。US检查结果:诊断乳腺良恶性肿瘤的准确度为82.17%,灵敏度为83.58%,特异度为89.35%;两种检查方法在鉴别诊断乳腺良性和恶性肿瘤方面具有统计学差异(P<0.05)。联合ABUS及US检查结果:诊断乳腺良恶性肿瘤的准确度为94.00%,灵敏度为90.91%,特异度为96.42%。结论:联合ABUS及US检查方法能明显提高乳腺良性与恶性肿瘤的鉴别诊断能力,在术前为临床医生提供准确的参考依据,值得在临床工作中广泛推广应用。Objective:To explore the ultrasonic features of benign and malignant breast tumors in automatic breast ultrasonography(ABUS)and conventional 2 D ultrasound(US),so as to improve the ultrasonic differential diagnosis ability of benign and malignant breast tumors and provide accurate reference information for clinical practice.Methods:Fifty patients with breast tumors confirmed by clinical pathology from August 2018 to October 2019 in our hospital were collected retrospectively.All patients underwent ABUS and US examination before operation.The ABUS and US image characteristics of breast tumors were analyzed and compared with the pathological results after clinical operation to compare the accuracy,sensitivity and specificity of ABUS and US in diagnosing breast malignant tumors.Results:Among the 50 patients with breast tumors,28 cases were benign breast tumors,including 18 cases of fibroadenoma,3 cases of galactocele and 5 cases of benign phyllodes tumor.22 cases of breast malignant tumors,including 12 cases of invasive ductal carcinoma,5 cases of invasive lobular carcinoma,3 cases of ductal carcinoma in situ and 2 cases of malignant phyllodes tumor.ABUS examination results:15 cases of breast tumors with"convergence sign"can be seen on coronal reconstruction images,12 cases are proved to be malignant by pathology,and 3 cases are proved to be benign by pathology.ABUS diagnosis accuracy for benign and malignant breast tumors is 89.23%,sensitivity is 87.61%,and specificity is 91.54%.US examination results showed that the accuracy,sensitivity and specificity of diagnosing benign and malignant breast tumors were 82.17%,83.58%and 89.35%,respectively.There were statistical differences between the two methods in differential diagnosis of benign and malignant breast tumors(P<0.05).Combined ABUS and US examination results showed that the accuracy,sensitivity and specificity of diagnosing benign and malignant breast tumors were 94.00%,90.91%and 96.42%,respectively.Conclusion:Combined ABUS and US examination methods can signifi
