摘 要:目的:比较单光子发射计算机断层显像(SPECT)/CT不同扫描参数序列对骨断层图像质量和定量分析的影响,优化骨断层图像参数序列。方法:使用9组参数序列对jaszczak模体进行扫描,根据图像质量选出较好的3组参数序列,再用这3组参数序列进行NEMA IEC模体扫描和40例患者扫描,选出最好的1组参数序列,分别以每帧采集时间为5 s、10 s和15 s,每个采集时间步进角度分别为3°、4°和6°,两两组合共9组参数序列,即3°5 s、4°5 s、6°5 s、3°10 s、4°10 s、6°10 s、3°15 s、4°15 s和6°15 s,总共9组序列,对Jaszczak模体进行SPECT/CT断层扫描,对扫描图像进行本底变异系数(CV)测量和主观图像质量评级,从中选出图像质量较好的3组参数序列,进行美国国家电气制造商协会(NEMA)国际电工委员会(IEC)模体扫描和临床实验。采用NEMA IEC模体扫描比较3组参数序列的对比噪声比(CNR)及比放射性活度比值(RRA)差异。40例平面骨扫描显示放射性浓聚者行3组参数序列的骨断层扫描,比较3组采集参数序列的骨断层图像质量评级差别。结果:Jaszczak模体试验结果显示3°5 s、3°10 s和4°10 s的3组扫描参数序列CV分别为0.0636、0.0633和0.0735,图像质量评级分别为4等2级、4等3级和4等2级,可以兼顾图像质量和临床扫描时间。3°5 s、3°10 s和4°10 s的3个序列在直径≥13 mm的5个热灶测得的CNR均值分别为34.30、45.56和36.79;RRA均值分别为0.637、0.663和0.646。3组扫描参数序列的骨断层扫描主观图像评级结果显示3°5 s、3°10 s和4°10 s序列的平均值分别为3.675、3.975和4.075,三者差异有统计学意义(x^2=16.137,P<0.05)。其中3°10 s和4°10 s两组序列比较无统计学差异,但均高于3°5 s序列,差异有统计学意义(Z=-2.683,Z=-3.578;P<0.05)。结论:3°10 s序列均可很好的兼顾总采集时长、图像质量和定量分析,适合作为断层骨显像的优选参数序列。Objective:To compare the effects of different scan parameter sequence of single photon emission computed tomography/computed tomography(SPECT/CT)on the image quality and quantitative analysis of bone cross-sectional image by comparing different parameter sequence effects on the quality and quantitative analysis.Methods:The parameter sequences of 9 groups were used to scan Jaszczak phantom.And the parameter sequences of 3 groups were selected according to image quality and they were used to scan NEMA IEC phantom and 40 patients,and then the best parameter sequence was picked out from them.The acquisition time of each frame adopted 5s,10s and 15s,and the step angles of each acquisition time adopted 3°,4°and 6°,respectively.There were the parameter sequences of 9 groups in pairs of the step angles and acquisition times,and they were 3°5 s,4°5 s,6°5 s,3°10 s,4°10 s,6°10 s,3°15 s,4°15 s and 6°15 s,respectively.And then they were used to implement SPECT/CT for Jaszczak phantom.The measurement of coefficient variance(CV)of background and quality rating of subjective image were implemented for scan image.The parameter sequences of 3 groups with better image quality were picked out from them for implementing National Electrical Manufacturers Association International Electro technical Commission(NEMA IEC)phantom scan and clinical experiment.The differences of contrast noise ratio(CNR)and ratio of radioactivity(RRA)of parameter sequences of 3 groups were compared by adopting NEMA IEC phantom.The patients with radioactive concentration underwent CT scan on bone of parameter sequences of 3 groups,and the differences of the rating of image quality of bone cross-section of the acquisition parameter sequence of 3 groups were compared.Results:The results of phantom experiment showed that the CV of the sequences of scan parameters of 3 groups(3°5s,3°10s and 4°10s)were 0.0636,0.0633 and 0.0735,and the grading of image quality were the 2th grade of the 4th level,the 3th grade of the 4th level and the 2th grade of the
