摘  要:目的:基于神经生长因子(NGF)-酪氨酸激酶受体A(TrkA)信号通路研究电针疗法治疗粪失禁兔动物模型的作用机制。方法:新西兰兔30只随机分为对照组、模型组和治疗组,每组10只,采用射频消融法制备粪失禁兔动物模型,造模成功后对照组和模型组不作任何干预,治疗组采用电针治疗。测定各组新西兰兔肛门静息压,采用HE染色观察各组肛门括约肌组织形态学变化,采用Western Blot检测肛门括约肌组织中NGF、TrkA蛋白表达量。结果:模型组新西兰兔肛门静息压显著低于对照组(P<0.05);治疗组新西兰兔肛门静息压显著高于模型组(P<0.05);治疗组新西兰兔肛门静息压显著低于对照组(P<0.05);模型组新西兰兔肛门括约肌组织中NGF、TrkA蛋白表达量显著低于对照组(P<0.05);治疗组新西兰兔肛门括约肌组织中NGF、TrkA蛋白表达量显著高于模型组(P<0.05);治疗组新西兰兔肛门括约肌组织中NGF、TrkA蛋白表达量显著高于对照组(P<0.05);HE染色显示,治疗组新西兰兔肛门括约肌组织形态结构较模型组明显改善,括约肌全层变厚,固有层结缔组织变薄,肌纤维变粗并趋于完整,肌纤维数量增多,横纹肌致密程度增加。结论:电针疗法可能通过激活NGF-TrkA信号通路对粪失禁兔动物模型起到改善作用,这可为临床治疗大便失禁提供理论依据。Objective:To investigate the mechanism of electro-acupuncture therapy in the treatment of fecal incontinence in rabbits based on neural growth factor-tyrosine kinase receptor A(NGF-TrkA)pathway.Methods:30 New Zealand rabbits were randomly divided into the control group,the model group and the treatment group,with 10 rabbits in each group.Fecal incontinence model was established through radiofrequency ablation.After successful modeling,the control group and the model did not received any intervention,whereas the treatment group was treated with electro-acupuncture.The anal resting pressure in each group was measured,the morphological changes of anal sphincter were observed by HE staining and the protein expressions of NGF and TrkA in anal sphincter was detected by Western Blot method in the three groups.Results:In terms of the anal resting pressure,it was lower in the model group than that in the control group(P<0.05);it was higher in the treatment group than that in the model group(P<0.05),but was lower than that in the control group(P<0.05).In terms of the protein expressions of NGF and TrkA,they were significantly lower in the model group than those in the control group(P<0.05);they were significantly higher in the treatment group than those in the model group and the control group(P<0.05).The HE staining showed that the morphological structure of anal sphincter was significantly improved in the treatment group compared to that in the model group;the whole layer of sphincter became thicker,the connective tissue in the lamina propria became thinner,the muscle fiber became thicker and more complete,the number of muscle fibers increased,and the density of striated muscle increased.Conclusion:Electro-acupuncture may improve fecal incontinence in rabbit model by activating NGF-TrkA signal pathway,which can provide theoretical basis for clinical treatment of fecal incontinence.
