火针点刺龈交穴联合耳穴贴压治疗气滞血瘀型痔疮临床研究

摘　　要：目的:探讨火针点刺龈交穴联合耳穴贴压治疗气滞血瘀型痔疮的疗效及对VAS评分影响。方法:选取本院门诊收治的气滞血瘀型痔疮患者100例,随机分为两组,各50例。对照组给予耳穴贴压治疗,治疗组在对照组基础上给予火针点刺龈交穴治疗,对比治疗后的临床疗效、痔核大小、肛门坠胀和痔核表面评分、VAS评分、5-HT水平及血清炎性因子水平。结果:经过治疗后,治疗组总有效率较高(P<0.05);两组患者痔核大小、肛门坠胀和痔核表面评分显著较低(P<0.05),且治疗组降低较多(P<0.05);两组VAS评分和5-HT水平显著降低(P<0.05),且治疗组降低较多(P<0.05);两组CRP、TNF-α和IL-6水平显著降低(P<0.05),且治疗组CRP、TNF-α和IL-6降低较明显(P<0.05)。结论:采用火针点刺龈交穴联合耳穴贴压治疗气滞血瘀型痔疮具有较好的临床疗效,能够降低VAS评分,值得在临床上推广应用。Objective:To investigate the curative effect of fire needling Yinjiao(DU28)combined with auricular plaster therapy in the treatment of hemorrhoids of qi stagnation and blood stasis and its influence to VAS score.Methods:100 patients with hemorrhoids of qi stagnation and blood stasis were randomly divided into the control group(n=50)and the treatment group(n=50).The control group was treated with auricular plaster therapy;on which basis,the treatment group was also treated with fire needling Yinjiao(DU28).The clinical efficacy,hemorrhoid size,anal bulge and hemorrhoid surface score,VAS score,5-HT level and serum inflammatory factor levels were compared between the two groups after the treatment.Results:The total effective rate of the treatment group was higher than that of the control group(P<0.05).Hemorrhoid size,anal bulge and hemorrhoid surface score were significantly improved in the two groups after the treatment(P<0.05),and the improvements in the treatment group were more significant(P<0.05).VAS score and 5-HT level were significantly decreased in the two groups(P<0.05),and the decreases in the treatment group were more significant(P<0.05).The levels of CRP,TNF-αand IL-6 were significantly decreased in both groups(P<0.05);of which the treatment group decreased more significantly(P<0.05).Conclusion:Fire needling DU28 combined with auricular plaster therapy has a good clinical effect and can reduce VAS score in the treatment of hemorrhoids of qi stagnation and blood stasis.The therapy is worthy of clinical promotion.
