摘　　要：目的探究小而密低密度脂蛋白(sd-LDL)与低密度脂蛋白的关系及在冠心病的发生发展中的作用,为冠心病临床早期诊断提供参考。方法择取2018年2月至2019年10月收治的冠心病患者100例作为A组,择取同期健康中心体检正常人100名作为B组,比较2组sd-LDL水平与低密度脂蛋白(LDL)指标、心脏彩色多普勒超声检测结果、D-二聚体(D-D)、超敏C-反应蛋白水平(hs-CRP)、纤维蛋白原(FIB)以及同型半胱氨酸(Hcy)水平。结果A组sd-LDL水平与低密度脂蛋白指标均高于B组(P<0.05);A组左心室舒张末内径、左心室收缩末内径值高于B组,左心室射血分数低于B组(P<0.05);A组D-D、hs-CRP、FIB以及Hcy水平要显著高于B组(P<0.05)。结论血清sd-LDL与低密度脂蛋白指标是冠心病发展发生的独立危险因素,血清小而密低密度脂蛋白水平与冠心病患者冠状动脉狭窄程度有关,做好指标检测对于冠心病的早期预测具有较高的临床价值,需要临床医学引起重视。Objective To investigate the correlation between the level of small and dense low-density lipoprotein and the indexes of low-density lipoprotein as well as the pathogenesis and development of coronary heart disease,so as to provide reference for the early diagnosis of coronary heart disease.Methods One hundred patients with coronary heart disease who were admitted and treated in our hospital from February 2018 to October 2019 were enrolled as group A,and the other 100 healthy subjects who underwent medical examination in our hospital at the same period were enrolled as group B.The levels of small and dense low-density lipoprotein and low-density lipoprotein indexes,and cardiac color Doppler ultrasound results as well as the levels of D-dimer(DD),high-sensitivity C-reactive protein(hs-CRP),fibrinogen(FIB),isotype cysteine(Hcy)were observed and compared between the two groups.Results The levels of small and dense low-density lipoprotein and low-density lipoprotein indexes in group A were significantly higher than those in group B(P<0.05).The levels of left ventricular end-diastolic diameter and left ventricular end-systolic diameter in group A were significantly higher than those in group B(P<0.05),however,the levels of left ventricular ejection fraction ingroup A were significantly lower than those in group B(P<0.05).And the levels of DD,hs-CRP,FIB,and Hcy in group A were significantly higher than those in group B(P<0.05).Conclusion The serum small and dense low density lipoprotein levels and low density lipoprotein indexes are independent risk factors for the pathogenesis and development of coronary heart disease,moreover,the serum small and dense low density lipoprotein levels are correlated with coronary artery stenosis degree,Therefor,the early detection of related indexes has higher clinical value for the early prediction of coronary heart disease,which needs to be paid much attention to.
