摘　　要：目的探讨关节镜下克氏针、钢丝张力带内固定治疗胫骨髁间嵴骨折的临床疗效。方法关节镜下克氏针、钢丝张力带内固定治疗23例胫骨髁间嵴骨折患者。记录骨折愈合情况。术后3、6个月及末次随访时,采用前抽屉试验及Lachman试验检测膝关节稳定性,采用Lysholm评分标准评价膝关节功能。结果患者均获得随访,时间8~30(14.0±2.5)个月。骨折均愈合。术后3、6个月及末次随访时,前抽屉试验及Lachman试验均阴性。Lysholm评分由术前的49~67(60.8±1.5)分提高到末次随访时的94~100(95.5±2.5)分。末次随访时,膝关节屈伸活动正常,患者主诉膝关节无不稳感。结论关节镜下克氏针、钢丝张力带内固定治疗胫骨髁间嵴骨折,创伤小,固定牢靠,患者能早期行关节功能锻炼,术后膝关节稳定性及活动度恢复满意。Objective To explore the clinical results of Kirschner wire combined with wire tension band internal fixation for treatment of tibial intercondylar eminence fracture under arthroscopy.Methods The 23 patients with tibial intercondylar eminence fractures were treated with arthroscopy Kirschner wire combined with wire tension band internal fixation.The fracture healing were recorded.At 3,6 months and the last follow-up,the anterior drawer test and Lachman test were used to evaluate the stability of knee joint.Lysholm score was used to evaluate the function of knee joint.Results All 23 patients were followed-up for 8~30(14.0±2.5)months.All the fractures were healed.The anterior drawer test and Lachman test were negative at 3,6 months and the last follow-up.Lysholm score increased from 49~67(60.8±1.5)points before operation to 94~100(95.5±2.5)points at the last follow-up.At the last follow-up,the flexion activities of knee joint were normal,and there was no instability in the knee joint.Conclusions Kirschner wire combined with wire tension band internal fixation for treatment of tibial intercondylar eminence fracture under arthroscopy has the advantages of less trauma,firm fixation,patients can exercise early and quickly restore the stability and movement function of knee joint after operation.
