摘  要:目的探讨卡格列净对老年2型糖尿病(T2DM)合并心力衰竭患者心功能及预后的影响。方法入选我院2018年1~12月收治的T2DM合并心力衰竭患者128例,随机分为治疗组(卡格列净)64例和对照组64例,随访1年,比较2组患者血糖控制情况(空腹血糖、餐后2h血糖、糖化血红蛋白)、心功能(B型钠尿肽、LVEF、左心室舒张末压、6min步行距离)、炎性因子及不良预后差异。结果治疗1年后,治疗组空腹血糖、餐后2h血糖、糖化血红蛋白、B型钠尿肽、炎性因子、左心室舒张末压低于对照组,而LVEF、6min步行距离高于对照组高(P<0.05,P<0.01)。治疗组不良预后事件发生率低于对照组(7.8%vs 21.9%,P<0.05)。结论卡格列净对T2DM合并心力衰竭患者血糖、心功能及预后均有一定的改善作用,作用机制可能与其降糖及抗炎性反应作用有关。Objective To study the effect of caglitazin on cardiac function and outcome in elderly type 2DM patients with heart failure(HF).Methods One hundred and twenty-eight type 2DM patients with HF admitted to our hospital in December 2018were randomly divided into caglitazin treatment group(n=64)and control group(n=64).The patients were followed up for 1year,during which their serum levels of fasting plasma glucose(FPG),2hpostprandial blood glucose(PBG),HbA1c,BNP,and LVEF,LVDDP,6min walking distance,inflammatory factors,outcome were compared.Results The serum levels of FPG,2hPBG,HbA1c,BNP,inflammatory factors and LVDDP were significantly lower while the LVEF was significantly higher and the 6 min walking distance was significantly longer in caglitazin treatment group than in control group after 1year of treatment(P<0.05,P<0.01).The incidence of adverse events was lower in caglitazin treatment group than in control group(7.8%vs 21.9%,P<0.05).Conclusion Caglitazin can improve the blood glucose,cardiac function and outcome in elderly type 2DM patients with HF by reducing their serum blood glucose level and inflammatory reactions.
