生精汤联合腹腔镜下精索静脉高位结扎术在精索静脉曲张性不育症中的应用价值研究

摘　　要：目的探究生精汤联合腹腔镜下精索静脉高位结扎术在精索静脉曲张性不育症中的应用价值。方法选取2019年1月至2020年1月广西医科大学附属武鸣医院收治的60例左侧精索静脉曲张性不育症患者作为研究对象,通过随机数字表法分为A组与B组,各30例,A组患者采用腹腔镜下精索静脉高位结扎术,B组在腹腔镜下精索静脉高位结扎术后连续服用生精汤治疗3个月。比较两组患者治疗前后的精液常规及血中促卵泡刺激素(follicle stimulating hormone,FSH)、促黄体生成素(luteinizing hormone,LH)和睾酮(testosterone,T)水平。结果B组治愈率与显效率均高于A组,无效率低于A组,两组治疗效果比较差异有统计学意义(P<0.05)。治疗后两组精液量、精液密度、精子成活率、前向运动精子百分比及正常形态百分比均较治疗前有所提高,其中治疗3个月后B组精液密度、精子成活率、前向运动精子百分比及正常形态百分比均明显高于A组(P<0.05)。治疗后,两组FSH与LH水平均较治疗前有所下降,T水平较治疗前提高(P<0.05);其中治疗3个月后B组FSH与LH水平显著低于A组,T水平显著高于A组,差异均有统计学意义(P<0.05)。结论生精汤联合腹腔镜下精索静脉高位结扎术治疗精索静脉曲张性不育症效果显著,可有效提高患者精液质量,显著降低FSH与LH水平,提高T水平。Objective To explore the application value of Shengjing Decoction combined with laparoscopic spermatic vein ligation in varicocele infertility.Methods 60 patients with left varicocele infertility treated in Wuming Hospital Affiliated to Guangxi Medical University from January 2019 to January 2020 were selected as the research object.Random number table method was used to divide them into group A and group B,30 cases in each group.Patients in group A underwent laparoscopic spermatic vein high ligation,group B took Shengjing Decoction for 3 months after laparoscopic spermatic vein high ligation.The semen routine and blood follicle stimulating hormone(FSH),luteinizing hormone(LH)and testosterone(T)levels were compared between the two groups before and after treatment.Results The cure rate and apparent efficiency of group B were higher than those of group A,and the inefficiency was lower than that of group A.The difference in treatment effect between the two groups was statistically significant(P<0.05).After treatment,semen volume,semen density,sperm survival rate,percentage of sperm moving forward and percentage of normal morphology were increased compared with before treatment;after 3 months of treatment,the semen density,sperm survival rate,forward movement sperm percentage and normal morphology percentage of group B were significantly higher than those of group A(P<0.05).After treatment,the FSH and LH levels of the two groups decreased compared with before treatment,and the T level increased compared with before treatment(P<0.05);after 3 months of treatment,the FSH and LH levels in group B were significantly lower than those in group A,and the T levels was significantly higher than that in group A(P<0.05).Conclusion Shengjing Decoction combined with laparoscopic spermatic vein ligation has a significant effect on the treatment of varicocele infertility,which can effectively improve the quality of semen,significantly reduce the levels of FSH and LH,and increase the level of T.
