摘 要:目的探讨890nm红外激光联合α-硫辛酸、依帕司他在2型糖尿病周围神经病变中的应用价值。方法选取2018年1月~2019年12月钦州市第一人民医院内分泌科住院的2型糖尿病合并糖尿病周围神经病变患者100例,按照随机数字表法分为两组,每组各50例。对照组给予糖尿病基础治疗,静脉滴注α-硫辛酸注射液联合依帕司他,观察组在对照组基础上加用890nm红外激光照射,比较两组患者治疗前后TCSS评分、血清超氧化物歧化酶(SOD)浓度变化、神经传导速度变化、四肢感觉阈值变化、有无不良反应发生。结果干预后观察组TCSS评分显著低于干预前及干预后对照组(P <0.05),治疗后观察组血清超氧化物歧化酶浓度显著高于治疗前及治疗后对照组(P <0.05),干预后观察组腓神经运动传导速度显著高于干预前及干预后对照组(P <0.05),两组均未出现明显不良反应。两组干预前后四肢感觉阈值无明显变化,差异无统计学意义(P> 0.05),观察组总有效率显著高于对照组(P <0.05)。结论针对2型糖尿病周围神经病变患者,在应用α-硫辛酸联合依帕司他治疗基础上使用890nm红外激光治疗,可有效改善局部神经功能,提高机体抗氧化能力,从而确保临床治疗效果,且治疗不良反应少,安全性高。Objective To investigate the application value of 890 nm infrared laser combined with alpha lipoic acid and epalrestat in peripheral neuropathy of type 2 diabetes mellitus(T2DM).Methods From January 2018 to December 2019,a total of 100 patients with T2DM complicated with diabetic peripheral neuropathy hospitalized in the department of endocrinology in the First People’s Hospital were randomly divided into the control group(n=50)and the observation group(n=50)by random number table.Patients in the control group were given basic treatment of diabetes,intravenous drip of alpha lipoic acid injection combined with epalrestat,and patients in the observation group were given 890 nm infrared laser irradiation on the basis of the control group.TCSS scores,changes in serum superoxide dismutase(SOD)concentration,nerve conduction velocity,changes in sensory threshold of limbs and adverse reactions(ADRs)were compared between the two groups before and after treatment.Results TCSS scores of the observation group after intervention was significantly lower than that of the control group before and after intervention(P<0.05),serum SOD concentration of the observation group after treatment was significantly higher than that of the control group before and after treatment(P<0.05),and peroneal nerve motor conduction velocity of the observation group after intervention was significantly higher than that of the control group before and after intervention(P<0.05).There was no obvious ADRs between the two groups.There was no change in the sensory threshold between the two groups before and after intervention(P>0.05).The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).Conclusion For patients with T2 DM peripheral neuropathy,treatment with 890 nm infrared laser on the basis of alpha lipoic acid combined with epalrestat can effectively improve the topical nerve function and enhance the body’s antioxidant capacity,thus ensure the clinical treatment efficacy,with few ADRs and h
