动脉瘤性蛛网膜下腔出血患者全身免疫炎症指数与Hunt-Hess分级的相关性

摘　　要：目的探究动脉瘤性蛛网膜下腔出血(aneurysmal subarachnoid hemorrhage,aSAH)患者入院时全身免疫炎症指数(systemic immune-inflammation index,SII)与Hunt-Hess分级的相关性。方法回顾性收集和分析2015年1月至2019年1月间接受治疗的261例aSAH患者的数据资料。根据患者入院时Hunt-Hess分级分成高级别组(4~5级,71例)和低级别组(1~3级,190例),通过单因素及多因素回归分析比较两组间SII的差异,通过构建受试者工作特征曲线(ROC)并计算曲线下面积(AUC)来评价SII对于aSAH级别的预测能力。结果共纳入261例患者,多因素Logistic回归分析显示SII(OR=1.001,95%CI:1.000~1.002,P=0.046)是Hunt-Hess分级的独立危险因素。ROC曲线分析显示,AUC为0.730(95%CI:0.657~0.803,P<0.001),表明SII对于aSAH的分级拥有较好的预测能力。结论SII是aSAH高分级的独立危险因素,可以作为判断病情严重程度的新型指标。Objective To investigate the relationship between systemic immune-inflammation index(SII)and Hunt-Hess grading in patients with aneurysmal subarachnoid hemorrhage(aSAH).Methods Clinical data of 261 patients with aSAH admitted in our institution between January 2015 and January 2019 were retrospectively reviewed.The relationship of SII and Hunt-Hess grate of patients on admission was analyzed.Results Multivariate Logistic regression analysis revealed that SII was an independent risk factor of high Hunt-Hess grading(OR=1.001,95%CI:1.000-1.002,P=0.046).ROC analysis revealed that area under the ROC curve(AUC)of SII in predicting high Hunt-Hess grade was 0.730(95%CI:0.657-0.803,P<0.001).Conclusion SII is associated with Hunt-Hess grade,it might serve as a biomarker for the severity of aSAH.
