摘　　要：外科介入治疗作为一项将传统外科和介入治疗相融合的新技术,不同于传统的先天性心脏病(先心病)经皮介入治疗,它是通过外科手术切口,经心脏、大血管或外周血管进行心血管疾病介入诊断及治疗。该技术主要面对的是较危重或不适合传统经皮介入治疗的患者,故手术难度较大,并发症相对较多,早期主要在各区域大医院开展,所以外科介入质控中心的工作主要集中于此类大医院。随着经验的积累和技术的推广,国内地市级医院也在逐渐开展外科介入治疗,截至2018年年底,中国心血管外科注册登记系统(CCSR)共有80家注册登记医院,登记病例数超过13 000例,病种分布以房间隔缺损最为常见,其次是室间隔缺损、动脉导管未闭和肺动脉瓣狭窄。如何加强对各个医疗中心的质量控制、以期为患者提供更加安全的治疗便成为了此类新技术的关键。Surgical intervention strategy is a novel approach integrating the traditional surgery and intervention methodologies,which is different from the traditional percutaneous intervention of congenital heart disease.Surgical intervention approach achieve the goal of diagnoses and treatment of cardiovascular disease through surgical incision on heart,large blood vessels and peripheral blood vessels.This technology is mainly applied for critical patients or patients who are not suitable for traditional percutaneous intervention,so this operation is more difficult and related with multiple complications.It is mainly performed among tertiary hospitals in various regions of China,so the work of quality control center for surgical intervention is mainly concentrated in these tertiary hospitals in the early stages.With the accumulation of experience and the promotion of this technology,surgical intervention approach is gradually applied among the domestic municipal hospitals.Till the end of 2018,80 hospitals shared their data in the“Chinese Cardiovascular Surgery Registry Database”(CCSR)and there were over 13000 registered cases treated with surgical intervention in this database,the most registered cases were atrial septal defect,followed by ventricular septal defect,patent arterial duct and pulmonary stenosis.The key to promote surgical intervention approach is to strengthen the quality control in individual medical center to provide more safe treatment for indicated patients with congenital heart disease.
