摘  要:目的对比分析二维半傅立叶单次激发快速自旋回波(2D-HASTE)序列和三维可变翻转角快速自旋回波(3D-SPACE)序列在大量腹腔积液患者磁共振胰胆管成像(MRCP)图像质量差异,探讨大量腹腔积液患者MRCP的成像技术。方法 100例临床怀疑胰胆管系统疾病且合并大量腹腔积液的患者分别行2D-HASTE和3D-SPACE MRCP检查,对2种序列的采集时间和图像质量进行统计学分析,比较这2种扫描方法的优缺点。结果 2D-HASTE、3D-SPACE序列采集时间分别为(64±7)s、(302±22)s,2种序列的采集时间差异有统计学意义(t=106.50,P<0.05)。2D-HASTE序列图像5分85例,占85%(85/100);4分12例,占12%(12/100);3分3例,占3%(3/100);1、2分0例。3D-SPACE序列图像5分44例,占44%(44/100);4分30例,占30%(30/100);3分18例,占18%(18/100);2分5例,占5%(5/100);1分3例,占3%(3/100)。2种序列的图像质量差异有统计学意义(Z=9.23,P<0.05)。结论相对3D-SPACE序列,2D-HASTE序列在大量腹腔积液患者MRCP图像质量更佳,采集时间更短,更适合大量腹腔积液患者MRCP的检查。Objective To compare and analyze the image quality differences of two-dimensional half-Fourier acquisition single-shot turbo spin-echo(2D-HASTE)sequence and three-dimensional sampling perfection with application-optimized contrasts by using different flip angle evolution(3D-SPACE)sequence in the magnetic resonance cholangiopancreatography(MRCP)of the patients with massive ascites,and to explore the imaging technology of the MRCP in the patients with massive ascites.Methods One hundred patients with clinically suspected pancreaticobiliary diseases and massive ascites were examined by 2D-HASTE and 3D-SPACE MRCP respectively.The acquisition time and image quality of the two sequences were analyzed statistically,and the advantages and disadvantages of the two scanning methods were compared.Results The acquisition time of 2D-HASTE and 3D-SPACE sequences were(64±7)s and(302±22)s respectively.There was significant difference between the two sequences(t=106.50,P<0.05).The image quality score of 2D-HASTE-MRCP:85 cases(85%)with 5 points,12 cases(12%)with 4 points,3 cases(3%)with 3 points,0 cases with 1 and 2 points.The image quality score of 3D-SPACE-MRCP:44 cases(44%)with 5 points,30 cases(30%)with 4 points,18 cases(18%)with 3 points,5 cases(5%)with 2 points,3 cases(3%)with 1 points.There was significant difference in image quality between the two sequences(Z=9.23,P<0.05).Conclusion Compared with 3D-SPACE sequence,2D-HASTE sequence has better MRCP image quality and shorter acquisition time in the patients with massive ascites,and is more suitable for MRCP examination in the patients with massive ascites.
