清上蠲痛汤加减治疗颈源性头痛临床研究

摘　　要：[目的]观察清上蠲痛汤治疗颈源性头痛的临床疗效。[方法]100例患者随机分为观察组和对照组,每组50例。对照组服用复方氯唑沙宗胶囊和颈复康颗粒治疗,观察组用清上蠲痛汤加减,汤剂内服,将煎煮后的中药渣在项枕部热敷。观察两组治疗前后疼痛程度(VAS)及颈椎活动度(ROM),测定治疗前后两组患者右侧椎动脉(RVA)、左侧椎动脉(LVA)和基底动脉(BA)的收缩期峰流速(Vs)、平均流速(Vm)。[结果]两组头痛程度评分VAS均较前改善,治疗组优于对照组(P<0.05);两组颈部活动度均优于治疗前,治疗组优于对照组(P<0.05);治疗组患者RVA、LVA和BA的Vs、Vm值均显著优于对照组(P<0.05)。两组总有效率比较差异有统计学意义(P<0.05)。[结论]清上蠲痛汤可显著改善颈源性头痛患者的头痛症状,改善颈部活动度和局部血流量。[Objective]To observe the clinical effect of Qingshanghuantong decoction on cervicogenic headache. [Methods]100 patients were randomly divided into observation group and control group, 50 cases in each group. The control group was treated with compound chlorzoxazone capsule and Jingfukang granule. In the observation group, qingshanghuantong decoction was added and reduced, and the decoction was orally taken, and the decoction dregs of traditional Chinese medicine were hot compressed on the neck. The degree of pain(VAS) and cervical mobility(ROM) were observed before and after treatment. The systolic peak velocity(Vs) and mean velocity(Vm) of right vertebral artery(RVA), left vertebral artery(LVA) and basilar artery(BA) were measured before and after treatment. [Results]The VAS score of headache degree in the two groups was improved, and the VAS score in the treatment group was better than that in the control group(P < 0.05). The neck mobility in both groups was better than that before treatment, and the cervical mobility of the treatment group was better than that of the control group(P < 0.05). The values of VS and VM of RVA, LVA and BA in the observation group were significantly higher than those in the control group(P < 0.05). The difference of total effective rate between the two groups was statistically significant.[Conclusion]Qingshanghuantong decoction can significantly improve the headache symptoms, neck activity and local blood flow of patients with cervical headache.
