基于《临床用药须知》成方制剂中地黄及其炮制品的合理应用研究

摘　　要：目的:加强地黄及其炮制品的炮制规范化,为临床合理安全应用提供参考。方法:基于2015年版《中华人民共和国药典临床用药须知》(以下简称《临床用药须知》)的中药成方制剂卷,归纳统计涉及地黄及其炮制品的成方制剂,按照功效、品种、配伍和临床科别进行分类。结果:2015年版《临床用药须知》中含地黄及其炮制品的成方制剂共计331个,其中按功效分,包括滋补肝肾、补血、养血凉血、清热养阴等;按品种分,包括地黄、生地黄、熟地黄、盐炙八地黄、蜜炙八地黄、熟地黄(盐制);按配伍分,包括君药制剂96个、臣药制剂81个、佐药制剂88个;按临床科别分,包括内科、外科、儿科、骨科、妇科等。结论:2015年版《临床用药须知》中地黄应用存在不规范性,主要包括书写不规范、炮制品的选择不适宜。因此,加强炮制标准化、规范化,为其临床应用提供安全、合理、适宜和有效的选择十分必要。Objective:To strengthen the standardization of the processing of Rehmannia glutinosa and its processed products,and to provide a reference for the reasonable and safe clinical application.Methods:Based on the 2015 edition of Chinese Pharmacopeia Guidelines for Clinical Drug Use(TCM prescription preparation volume),the set prescription preparations containing Rehmannia glutinosa and its processed products were summarized and classified according to efficacy,variety,compatibility of medicines,and clinical department.Results:A total of 331 set prescription preparations containing Rehmannia glutinosa and its processed products are included in the 2015 edition of Chinese Pharmacopeia Guidelines for Clinical Drug Use,of which,according to efficacy include tonifying liver and kidney,replenishing blood,nourishing blood and cooling blood,clearing heat and nourishing yin.Classified by varieties include Rehmannia glutinosa,Radix Rehmanniae,prepared Radix Rehmanniae,stir-frying with salt-water,stir-frying with honey,processing with salt-water.According to the compatibility of medicines,96 as sovereign drug,81 as minister drug,and 88 as assistant drug.Classified by clinical department include internal medicine,surgery,pediatrics,orthopedics,gynecology,etc.Conclusion:Some application of Rehmannia glutinosa is not standardized in the 2015 edition of Chinese Pharmacopeia Guidelines for Clinical Drug Use,mainly include nonstandard writing and inappropriate selection of processed products.Therefore,it is necessary to strengthen the standardization of processing of Rehmannia glutinosa to provide safe,reasonable,appropriate and effective choices for its clinical application.
