摘 要:目的探讨替加环素联合比阿培南对多重耐药菌感染重症肺炎患者血气指标及血清炎性因子水平的影响。方法选取江南大学附属医院2018年2月至2019年10月收治的50例多重耐药菌感染重症肺炎患者,将其以随机数字表法分为对照组和试验组,每组25例。予以对照组患者比阿培南治疗,试验组患者在对照组的基础上增加替加环素治疗,两组患者均治疗1周。比较两组患者治疗1周后感染控制效果;比较治疗前、治疗1周后两组患者血清炎性因子水平;比较治疗前、治疗1周后两组患者DO2、DO2I、PaCO2、PaO2、SaO2水平。结果治疗1周后,试验组患者的感染控制总有效率为96.00%,高于对照组患者的64.00%;与治疗前比,治疗1周后两组患者PaCO2水平及血清TNF-α、IL-6、PCT、hs-CRP水平均降低,且试验组低于对照组;治疗1周后两组患者DO2、DO2I、PaO2、SaO2水平均升高,且试验组高于对照组(均P<0.05)。结论替加环素联合比阿培南治疗多重耐药菌感染重症肺炎,可有效减轻机体炎症反应,改善患者氧代谢情况,提高感染控制效果。Objective To investigate the effects of tigecycline combined with biapenem on blood gas indexes and serum inflammatory factors in patients with severe pneumonia infected by multi-drug resistant bacteria.Methods A total of 50 patients with severe pneumonia infected by multi-drug resistant bacteria admitted to the Affiliated Hospital of Jiangnan University from February 2018 to October 2019 were selected and divided into the control group and the experimental group with 25 patients in each group according to the random number table method.Patients in the control group were treated with biapenem,and patients in the experimental group were treated with tigecycline and biapenem,patients in both groups were treated for 1 week.The infection control effect 1 week after treatment was compared between the two groups;the serum levels of inflammatory factors of patients were compared between the two groups before treatment and 1 week after treatment;DO2,DO2I,PaCO2,PaO2 and SaO2 levels of patients were compared between the two groups before treatment and 1 week after treatment.Results 1 week after treatment,the total effective rate of infection control in the experimental group was 96.00%,which was higher than 64.00%in the control group;compared with that before treatment,PaCO2 level and serum TNF-α,IL-6,PCT and hs-CRP levels in both groups decreased 1 week after treatment,and the experimental group was lower than the control group;the levels of DO2,DO2I,PaO2 and SaO2 in both groups increased 1 week after treatment,and the experimental group was higher than the control group(all P<0.05).Conclusion Tigecycline combined with biapenem in the treatment of severe pneumonia caused by multidrug-resistant bacteria can effectively reduce inflammation,improve oxygen metabolism and improve infection control effect.
