摘 要:目的检测视网膜脱离硅油填充术后房水中单核细胞趋化蛋白(MCP-1)与转移生长因子-β2(TGF-β2)的浓度,并进一步分析其与术后继发性青光眼的关系。方法选取2018年6月至2019年10月期间在南通大学第二附属医院因孔源性视网膜脱离行玻璃体切除联合硅油填充术的患者,入选病例无原发性青光眼或葡萄膜炎等免疫相关性眼病。硅油填充期间发生继发性青光眼者纳入第I组:共28例(28只眼);未发生继发性青光眼者纳入第2组:共33例(33只眼);年龄相关性白内障患者30例(30只眼)作为对照组纳入第3组。所有患者在视网膜复位术中、超声乳化白内障术中及硅油取出术中用1 ml无菌注射器抽取未稀释房水0.1~0.2 ml,放置于Eppendorf管中编号标记,-80℃冰箱冻存备用。使用Elisa法测定房水标本中的MCP-1、TGF-β2含量。结果第I1组视网膜复位术中房水MCP-1含量平均为(926.68±345.24)pg/ml,TGF-β2为(1107.11±344.31)pg/ml。取油时间3~8个月,平均(6.12±2.14)个月,高眼压时程1~6个月,平均4.8个2月,最高眼压24~58 mmHg,平均(41.35±8.23)mmHg。硅油取出术中房水MCP-1含量平均为(1566.14±380.84)pg/ml,TGF-β2为(1847.32±403.90)pg/ml。第2组视网膜复位术中房水MCP-1含量平均为(995.48±332.27)pg/ml,TGF-β2为(1182.16±330.80)pg/ml。取油时间6~15个月,平均(8.54±3.37)个月。硅油取出术中房水MCP-1含量平均为(1012.87±439.43)pg/ml,TGF-β2为(1222.22±605.51)pg/ml。第3组白内障术中房水MCP-1含量平均为(728.63±353.93)pg/ml,TGF-β2为(609.53±225.96)pg/ml。第1、2组视网膜复位术中房水MCP-1、TGF-β2含量显著高于第3组(均P<0.05),第1组在硅油取出术中房水MCP-1与TGF-β2浓度显著高于第2组(均P<0.001)。第1组中最高眼压与MCP-1及TGF-β2浓度之间均呈直线正相关,并且MCP-1与TGF-β2之间也成正相关。结论硅油填充术后的继发性青光眼房水中MCP-1和TGF-β2浓度显著升高,并且与眼压成正相关,可能�Objective To measured the concentrations of MCP-1 and TGF-β2 in aqueous humor after silicone oil tamponade for retinal detachment,and assessed the relationships betwee MCP-1,TGF-β2 levels and the risk of secondary glaucoma.Methods Patients with retinal detachment who underwent vitrectomy combined with silicone oil tamponade at the Nantong first people's hospital from June 2018 to October 2019 were included in this study.Inclusion criteria included no history of primary glaucoma,uveitis or other immune-related eye diseases.A total of 28 patients(28 eyes)with secondary glaucoma during the silicone oil tamponade process were included in group I,33 patients(33 eyes)without secondary glaucoma were included in group II,30 patients(30 eyes)with age-related cataract were included in group III as the control group.During the surgery,0.1 to 0.2ml aqueous humor was extracted with a 1 ml sterile syringe and placed in the Eppendorf tube and cryopreserved at-80℃refrigerator.The concentrations of MCP-1 and TGF-β2 in the aqueous humor samples were determined by Elisa.Results The mean concentration of MCP-1 in group I was 926.68±345.24pg/ml,and TGF-β2 was 1107.11±344.31pg/ml.The silicone oil removal time ranged from 3 to 8 months,with an average of 6.12±2.14 months.The duration of high intraocular pressure ranged from 1 to 6 months,with an average of 4.82 months,and the highest intraocular pressure was 24 to 58 mmHg,with an average of 41.35±8.23 mmHg.During silicone oil removal,the mean MCP1 and TGF-β2 concentration was 1566.14±380.84 pg/ml and 1847.32±403.90 pg/ml,respectively.For group II,the mean MCP-1 and TGF-β2 concentration during the retinal reattachment procedure was 995.48±332.27 pg/ml and 1182.16±330.80 pg/ml,respectively.The oil removal time ranged from 6 to 15 months with an average of 8.54±3.37 months.During silicone oil removal,the mean MCP-1 concentration was 1012.87±439.43 pg/ml and TGF-β2 was 1222.22±605.51pg/ml.In group III,the mean MCP1 and TGF-β2 concentration was 728.63±353.93 pg/ml and
