角膜基质透镜染色联合纤维蛋白胶在角膜溃疡穿孔修复中应用的临床观察

摘　　要：目的观察应用飞秒激光小切口基质透镜取出术(SMILE)术中的弃用角膜基质透镜染色后联合纤维蛋白胶治疗角膜穿孔患者的手术效果。方法搜集我院2018年2月至2019年7月角膜溃疡并穿孔的手术患者16例(16只眼),术中采用多层SMILE基质透镜或多层透镜加羊膜进行溃疡创面修补,术中应用纤维蛋白胶粘合多层移植组织后缝合,并用吲哚青绿进行透镜染色以提高辨识度。结果所有患者术中操作顺利,术后均在1周内完全上皮化,未见移植物融解等并发症。结论角膜基质透镜移植可用于各种角膜溃疡并穿孔患者的手术治疗,联合纤维蛋白胶和吲哚箐绿染色,可方便手术操作,提高手术成功率。Objective To observe the surgical effect of the treatment of corneal perforation with staining corneal stromal lenticules combined with fibrin glue in the SMILE operation. Methods Sixteen patients( 16 eyes) with corneal ulcer and perforation in our hospital from February 2018 to July 2019 were enrolled. The ulcer wounds were repaired with multi-layer corneal stromal lenticules or multi-layer lenticules combined with amniotic membrane during the operation,fibrin glue was applied to adhere the multilayer grafts. The wounds were sutured,and indocyanine green staining was used to improve the identification. Results All patients underwent the operation smoothly,and the epithelialization was complete within 1 week after the operation. No complications such as graft fusion was observed. Conclusions Corneal stromal lenticules transplantation,combined with fibrin glue and indocyanine green staining,can be used in the treatment of patients with corneal ulceration and perforation. This procedure can facilitate the operation and improve the success rate of operation.
