鼻内镜下鼻中隔矫正术后不同鼻腔填塞材料的填塞效果及舒适度对比研究

摘　　要：目的:观察凡士林纱条、膨胀海绵和明胶海绵对鼻内镜下鼻中隔矫正术后患者的填塞效果及舒适度的影响。方法:选取2017年10月-2019年10月来本院治疗的144例鼻中隔偏曲患者,按照随机数字表法分为对照组、观察组A和观察组B,每组各48例。三组均行鼻内镜下鼻中隔矫正术,术后对照组采用凡士林纱条,观察组A采用膨胀海绵,观察组B采用明胶海绵填塞止血。比较三组患者填塞期出血量、填塞时的难易程度和疼痛程度、填塞后的舒适度;术后随访4周,评价三组患者临床疗效与并发症发生情况。结果:与观察组A和对照组比较,观察组B的填塞期出血量、填塞难易程度和填塞疼痛程度低(P<0.05)。观察组B的生理、心理评分和舒适度总分高于与观察组A(P<0.05),观察组B的生理、心理评分、环境评分和舒适度总分高于对照组(P<0.05)。术后第4周,观察组B的总有效与对照组及观察组A比较,差异有统计学意义(P<0.05)。术后4周内,观察组B的术后并发症发生情况与对照组比较,差异有统计学意义(P<0.05)。结论:鼻内镜下鼻中隔矫正术后应用明胶海绵填塞鼻腔,可显著提高患者的舒适度,提高填塞效果和临床疗效,减少术后并发症的发生,值得临床推广应用。Objective:To observe the effect of Vaseline gauze,expanded sponge and gelatin sponge on the filling effect and comfort of patients with nasal septum correction under nasal endoscope.Method:A total of 144 patients with nasal septum deviation treated in our hospital from October 2017 to October 2019 were selected and divided into control group,observation group A and observation group B according to the random number table method,with 48 cases in each group.All the patients in the three groups received nasal septum correction under nasal endoscope.Vaseline gauze was used in the control group,expanded sponge was used in the observation group A,and gelatin sponge was used in the observation group B to stop bleeding.The amount of bleeding,the degree of difficulty and pain,and the degree of comfort were compared among the three groups.Result:Compared with the observation group A and control group,the amount of blood loss,the degree of tamping difficulty and the degree of tamping pain in the tamping period in the observation group B were lower (P<0.05).The physiological and psychological scores and total comfort scores of observation group B were higher than those of observation group A (P<0.05),and the physiological and psychological scores,environmental scores and total comfort scores of observation group B were higher than those of control group (P<0.05).4 weeks after the operation,the total effectiveness of group B was significantly different from that of the control group and group A (P<0.05).Within 4 weeks after surgery,the postoperative complications in the observation group B were significantly different from those in the control group (P<0.05).Conclusion:Using gelatin sponge to fill the nasal cavity after the nasal septum correction under nasal endoscope can significantly improve the comfort of patients,improve the filling effect and clinical effect,reduce the occurrence of postoperative complications,and is worthy of clinical application.
