Effects of Inhalation Drugs Combined with NPPV on Nutritional Status and Expression of CC16 and SP-D in COPD Patients Complicated with Respiratory Failure

摘  要:目的:探讨吸入药物联合无创正压通气(noninvasive positive pressure ventilation,NPPV)对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)合并呼吸衰竭患者营养状况及Clara细胞蛋白(CC16)、肺表面活性蛋白D(SP-D)表达的影响。方法:选取2018年1月-2019年1月于本院呼吸内科住院治疗的COPD合并呼吸衰竭患者50例,按照随机数字表法将其分为观察组和对照组,各25例。对照组吸入布地奈德福莫特罗吸入粉雾剂治疗,观察组在对照组基础上联合NPPV。比较两组治疗前后动脉血气指标、圣乔治呼吸问卷(St.George’s respiratory questionnaire,SGRQ)评分、改良英国医学研究学会呼吸困难指数(modified British medical research council,mMRC)、6 min步行试验(6-minute walk test,6-MWT)、营养指标、CC16及SP-D表达水平。结果:治疗后,观察组SGRQ评分与mMRC分级均低于对照组,而6-MWT高于对照组(P<0.05)。治疗后,观察组动脉血氧分压(PaO2),动脉血氧饱和度(SaO2)及氧合指数(OI)均高于对照组,而动脉二氧化碳分压(PaCO2)低于对照组(P<0.05)。治疗后,观察组体质指数(BMI)、血清总蛋白(TP)与血清白蛋白(ALB)水平均高于对照组(P<0.05)。治疗后,观察组CC16表达水平高于对照组,而SP-D表达水平低于对照组(P<0.05)。结论:吸入药物联合NPPV可有效改善COPD合并呼吸衰竭患者的症状与营养状况,增加CC16表达量、降低SP-D表达量,值得临床推广应用。Objective:To investigate the effect of inhalation drugs combined with noninvasive positive pressure ventilation (NPPV) on the nutritional status and the expression of Clara cell protein (CC16) and lung surfactant protein D (SP-D) in patients with chronic obstructive pulmonary disease (COPD) complicated with respiratory failure.Method:A total of 50 COPD patients complicated with respiratory failure hospitalized in the respiratory department of our hospital from January 2018 to January 2019 were selected.They were divided into observation group and control group according to random number table method,with 25 cases in each group.The control group was treated with Budesonide and Formoterol.The observation group was combined with NPPV on the basis of the control group.The arterial blood gas index,St.George’s respiratory questionnaire (SGRQ) score,modified British medical research council (mMRC),6-minute walk test (6-MWT),nutritional index,CC16 and SP-D expression levels were compared between the two groups before and after treatment.Result:After treatment,the SGRQ score and mMRC grade of the observation group were lower than those of the control group,while the 6-MWT of the observation group was higher than that of the control group (P<0.05).After treatment,partial arterial oxygen pressure (PaO2),arterial oxygen saturation (SaO2) and oxygenation index (OI) in the observation group were higher than those in the control group,while partial arterial carbon dioxide pressure (PaCO2) was lower than that in the control group (P<0.05).After treatment,the body mass index (BMI),serum total protein (TP) and serum albumin (ALB) levels in the observation group were higher than those in the control group (P<0.05).After treatment,the CC16 expression level in the observation group was higher than that in the control group,while the SP-D expression level was lower than that in the control group (P<0.05).Conclusion:Inhalation drugs combined with NPPV can effectively improve the symptoms and nutritional status of COPD patients compli
