摘　　要：目的:探讨罗哌卡因复合不同剂量舒芬太尼连续硬膜外镇痛在分娩镇痛患者中的应用效果。方法:选取2019年1-12月间在本院产科分娩的80例产妇,随机分为低剂量组(27例),中剂量组(27例)和高剂量组(26例),低剂量组患者给予0.1%罗哌卡因+0.3 μg/mL舒芬太尼,中剂量组患者给予0.1%罗哌卡因+0.4 μg/mL舒芬太尼,高剂量组患者给予0.1%+0.5 μg/mL舒芬太尼。对比三组产妇的镇痛效果以及分娩情况。结果:给药后10、30、60 min和宫口开6 cm、宫口全开低剂量组VAS评分显著高于中、高剂量组(P<0.05),中剂量组第一产程时间显著短于低、高剂量组(P<0.05);低剂量组自然分娩率显著低于中、高剂量组(P<0.05);中、高剂量组使用缩宫素率显著少于低剂量组(P<0.05);三组新生儿出生后1、5、10 min的Apgar评分以及产妇不良反应发生率比较,差异均无统计学意义(P>0.05)。结论:采用罗哌卡因联合中剂量舒芬太尼对产妇分娩中镇痛作用疗效优于低、高剂量组,其疗效确切安全,值得大力推广。Objective:To investigate the effect of Ropivacaine combined with Sufentanil in continuous epidural analgesia for labor analgesia.Method:A total of 80 parturients were randomly divided into low dose group (27 cases),middle dose group (27 cases) and high dose group (26 cases),low dose group (0.1% ropivacaine + 0.3 μg/mL Sufentanil),middle dose group (0.1% Ropivacaine + 0.4 μg/mL Sufentanil,high dose group (0.1% + 0.5 μg/mL Sufentanil).Result:The VAS score of the low dose group was significantly higher than that of the middle and high dose group (P<0.05),the first labor time of the middle dose group was significantly shorter than that of the low and high dose groups (P<0.05),the natural delivery rate of the low dose group was significantly lower than that of the middle and high dose groups (P<0.05),the oxytocin usage rate of the low dose group was significantly higher than that of the middle and high dose groups (P<0.05).There was no significant difference in Apgar scores at 1,5 and 10 minutes after birth and the incidence rate of adverse reactions (P>0.05).Conclusion:The analgesic effect of Ropivacaine combined with middle dose Sufentanil is better than that of low and high dose groups in parturition,which is safe and effective and worthy of promotion.
