摘  要:目的:探讨经湿化高流量鼻导管通气对婴儿重症肺炎辅助治疗效果。方法:选取2018年1月-2019年12月入住本院的138例重症肺炎患儿。将其随机分为对照组A、B及研究组,各46例。其中对照组A仅采用常规鼻导管吸氧治疗,对照组B施以无创持续气道正压通气治疗,研究组采用经湿化高流量鼻导管通气治疗。比较的三组治疗效果、呼吸、心率水平变化情况、临床症状消失时长及住院时长、血气分析各指标值变化情况。结果:研究组的治疗有效率高于对照组A、B,差异均有统计学意义(P<0.05)。治疗后,对照组A、B的呼吸、心率水平均高于研究组,差异均有统计学意义(P<0.05)。对照组A、B的肺部啰音消失时长、呼吸困难消失时长、住院时长均长于研究组,差异均有统计学意义(P<0.05)。治疗后,对照组A、B的动脉血氧饱和度、动脉血氧分压都低于研究组,差异均有统计学意义(P<0.05)。结论:在治疗重症肺炎患儿的过程中,经湿化高流量鼻导管通气治疗效果显著,可在临床上广泛推广应用。Objective:To investigate the effect of humidified high flow nasal catheter ventilation on adjuvant treatment of infant severe pneumonia.Method:A total of 138 children with severe pneumonia admitted to our hospital from January 2018 to December 2019 were selected.They were randomly divided into control group A,control group B and study group,with 46 cases in each group.Control group A was only treated with conventional nasal catheter oxygen therapy,control group B was treated with noninvasive continuous positive airway pressure ventilation,and study group was treated with humidified high flow nasal catheter ventilation.Treatment effect,changes in respiration and heart rate levels,duration of disappearance of clinical symptoms and length of stay in hospital,and changes in blood gas analysis of each indicator were compared among the three groups.Result:The treatment efficiency of the study group was higher than that of the control group A and B,and the differences were statistically significant (P<0.05).After treatment,the respiration and heart rate levels of the control group A and B were higher than those of the study group,with statistically significant differences (P<0.05).The vanishing time of rales,the vanishing time of dyspnea and the hospitalization time in the control group A and B were all longer than those in the study group,the differences were statistically significant (P<0.05).After treatment,the arterial oxygen saturation and partial pressure of arterial blood oxygen in control group A and B were lower than those in study group,the differences were statistically significant (P<0.05).Conclusion:In the treatment of children with severe pneumonia,the effect of humidified high flow nasal catheter ventilation is significant,which can be widely used in clinical practice.
