血浆同型半胱氨酸在慢性阻塞性肺疾病急性发作中的应用价值

摘　　要：目的研究血浆同型半胱氨酸(Hcy)检测在慢性阻塞性肺疾病急性发作(Acute exacerbation of chronic obstructive pulmonary disease,AECOPD)中的临床意义。方法选取2017年6月至2018年6月本院收治的AECOPD患者105例作为AECOPD组,另选取同期健康对照者80名作为对照组,检测血浆Hcy水平;AECOPD组随机分为常规治疗组(n=50)和加服叶酸治疗组(叶酸治疗组,n=55),比较两组在住院治疗后3个月及6个月Hcy水平及肺功能情况。结果 AECOPD组患者Hcy水平均较同期对照组明显升高(P<0.01)。治疗后,AECOPD组Hcy水平较治疗前有所下降,且叶酸治疗组下降更明显(P<0.05,P<0.01),叶酸治疗组在治疗后3、6个月的FEV1及FEV1%改善更明显(P<0.01)。结论 AECOPD患者Hcy水平较高,经治疗后Hcy水平明显下降,且服用叶酸后肺功能改善明显。Objective To study the clinical significance of homocysteine(Hcy)in acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods From June 2017 to June 2018,105 cases AECOPD patients were selected as AECOPD group,and 80 healthy controls were selected as control group.The contents of Hcy in plasma were detected in all patients.The AECOPD group was randomly divided into folic acid treatment group and routine treatment group.Hcy levels and pulmonary function were compared between the two groups at 3 months and 6 months after hospitalization.Results The average levels of Hcy in AECOPD group were significantly higher than that in the control group(P<0.01).After treatment,Hcy level in AECOPD group decreased compared with that before treatment,and the decrease in folic acid treatment group was more obvious(P<0.05,P<0.01).The improvement of FEV1 and FEV1%in folate group was more obvious at 3 and 6 months after treatment.Conclusion The level of Hcy in AECOPD patients was higher than that in AECOPD patients.After treatment,Hcy level decreased significantly,and pulmonary function improved significantly after taking folic acid.
