摘 要:目的探讨3.0T磁共振动态增强联合弥散加权成像对前列腺癌的诊断价值。方法回顾性分析本院2017年12月至2019年12月收治的80例前列腺疾病患者的临床资料,将前列腺穿刺活检结果为前列腺增生(Benign Prostatic Hyperplasia,BPH)的患者作为良性组(n=47),将前列腺穿刺活检结果为前列腺癌的患者作为恶性组(n=33),两组患者均给予3.0T磁共振动态增强联合弥散加权成像诊断,比较两组患者在磁共振动态增强扫描及磁共振弥散加权成像方面的影像特征。结果两组峰值信号强度(Signal Intensity of Peak,SIpeak)比较差异无统计学意义;恶性组峰值时间(peak time,Tmax)低于良性组,信号增强率(singal enhancement ratio,SER)、最大增强斜率(Maximum Slope of Increase,MSI)高于良性组,差异有统计学意义(P<0.05);在b值为1 000/1 500/2 000 s/mm^2时,恶性组的ADC值均低于良性组,差异有统计学意义(P<0.05);恶性组时间-信号强度曲线(The time-intensity curve,TIC)中Ⅰ型、Ⅱ型占比低于良性组,Ⅲ型占比高于良性组,差异有统计学意义(P<0.05)。结论 3.0T磁共振动态增强联合弥散加权成像可以客观辨别前列腺疾病的良恶性病变及诊断前列腺癌,为临床早期筛查前列腺癌及治疗方案的制定提供客观参考依据。Objective To investigate the diagnostic value of 3.0 T magnetic resonance dynamic enhancement combined with diffusion weighted imaging for prostate cancer. Methods The clinical data of 80 patients with prostate disease admitted to our hospital from December 2017 to December 2019 were retrospectively analyzed. The patients whose prostate biopsy results were Benign Prostatic Hyperplasia(BPH) were regarded as the benign group(n=47), patients whose prostate biopsy results were prostate cancer were regarded as malignant group(n=33). Both groups of patients were diagnosed with 3.0 T MRI dynamic enhancement combined with diffusion weighted imaging. The imaging characteristics of the two groups’ patients on MRI dynamic enhancement scan and MRI diffusion weighted imaging were compared. Results The signal intensity of peak(Signal Intensity of Peak, SIpeak) of the two groups was compared, the difference was not statistically significant;the peak time(Tmax) of the malignant group was lower than that of the benign group, and the signal enhancement ratio(SER) and Maximum Slope of Increase(MSI) were higher than that of the benign group, and the difference was statistically significant(P<0.05);when the b value was 1 000/1 500/2 000 s/mm^2, the ADC value of the malignant group was lower than that of the benign group, and the difference was statistically significant(P<0.05);in the time-intensity curve(TIC) of the malignant group, the proportion of type Ⅰ and type Ⅱ was lower than that of the benign group, and the proportion of type Ⅲ was higher than that of the benign group, the difference was statistically significant(P<0.05). Conclusion 3.0 T MRI dynamic enhancement combined with diffusion weighted imaging can objectively distinguish benign and malignant lesions of prostate disease, and diagnose prostate cancer, which can provide an objective reference for early screening of prostate cancer and the formulation of treatment plans in clinical practice.
