摘　　要：目的:探讨盐酸氢吗啡酮用于骨科术后硬膜外自控镇痛的效果。方法:2019年4月-2020年4月收治骨科手术患者94例,分为两组,各47例。对照组应用盐酸吗啡联合罗哌卡因镇痛;观察组应用盐酸氢吗啡酮联合罗哌卡因镇痛。比较两组疼痛程度、镇静程度、疼痛介质及镇痛效果。结果:观察组术后VAS评分低于对照组,Ramsay评分高于对照组,差异均有统计学意义(P<0.05);观察组术后36 h疼痛介质水平低于对照组,差异有统计学意义(P<0.05);观察组镇痛效果优良率高于对照组,差异有统计学意义(P<0.05)。结论:盐酸氢吗啡酮用于骨科术后硬膜外自控镇痛的效果显著,可明显减轻患者疼痛,提高镇静效果,降低疼痛介质水平。Objective:To explore the effect of hydromorphone hydrochloride on postoperative epidural self-controlled analgesia after orthopedics.Methods:From April 2019 to April 2020,94 patients with orthopedic surgery were selected,they were randomly divided into the two groups with 47 cases in each group.The control group was given morphine hydrochloride combined with ropivacaine for analgesia.The observation group was given hydromorphone hydrochloride combined with ropivacaine for analgesia.The pain degree,sedation degree,pain medium and analgesic effect were compared between the two groups.Results:The VAS score of the observation group after surgery was lower than that of the control group,and the Ramsay score was higher than that of the control group,the differences were statistically significant(P<0.05).The pain medium level in the observation group was lower than that in the control group at 36 hours after operation,the differences were statistically significant(P<0.05).The excellent and good rate of analgesic effect in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The effect of hydromorphone hydrochloride in patient-controlled epidural analgesia after orthopedic surgery is significant,which can significantly reduce the pain of patients,improve the sedative effect and reduce the level of pain medium.
