丙戊酸血药浓度监测结果的案例回顾性分析

摘　　要：目的:对我院近年来的丙戊酸血药浓度监测结果进行回顾性分析,讨论导致个体间差异的可能因素。方法:采用均相酶放大免疫法(EMIT)对333例次患者的血药浓度进行检测,对结果和患者资料进行回顾性分析,并结合案例说明血药浓度监测的重要性。结果:有183例次血药浓度监测结果在目标血药浓度(50~100μg/mL),仅占总例次的54.96%;131例次浓度结果低于最低有效血药浓度,占总例次的39.34%;19例次浓度结果高于最大有效血药浓度,占总例次的5.71%。就诊科室主要为儿科(52.55%)、神经内科(18.32%)、ICU(8.41%)、PICU(4.80%)和神经外科(4.80%)。其血药浓度与性别无显著相关性,但与患者的年龄存在一定的相关性。通过一个典型案例,药师借助血药浓度监测发现了临床中的药物相互作用,干预后效果明显。结论:服用丙戊酸期间应对其进行监测,以保障用药的安全。Objective:To analyze the plasma concentration of valproic acid in our hospital and to explore the factors that may affect its concentrations.Methods:The plasma concentration of valproic acid was monitored by enzyme multiplied immunoassay technique(EMIT)in 333 patients,the results were retrospectively analyzed,and the importance of concentration monitoring is illustrated by combined with a case study.Results:There were 183 cases of plasma concentration monitoring results in the effective plasma concentration range(50~100μg/mL),only accounts for 54.96%;131 cases lower than the minimum effective plasma concentration,accounts for 39.34%;19 cases of plasma concentration monitoring results than the maximum effective plasma concentration,accounts for 5.71%.The department was mainly pediatric(52.55%),neurology(18.32%),ICU(8.41%),PICU(4.80%)and neurosurgery(4.80%).The plasma concentration is not related to the sex,but there is a certain correlation with the age.In a typical case,pharmacists found the drug interaction in clinical with the help of drug concentration monitoring,and the effect was obvious after intervention.Conclusion:Therapeutic drug monitoring should carry out while taking valproic acid to ensure the safety of the patients.
