改良双腔球囊导管配合阴道超声引导造影的临床应用价值

摘　　要：目的探讨改良双腔球囊导管配合阴道超声引导造影检查,对畸形子宫及输卵管性不孕诊断的优势。方法收集2018年2月至2020年2月间在牡丹江市妇幼保健院生殖科就诊的子宫输卵管造影(hysterosalpingography,HSG)不孕不育患者1262例,其中742例行常规双腔球囊导管造影,520例行改良双腔球囊导管配合阴道超声引导造影。比较两组插管困难原因及插管困难率,观察正常子宫及畸形子宫的造影形态,分析对子宫畸形及输卵管阻塞诊断准确性的优势。结果常规双腔球囊导管造影插管困难率14.01%,与常规双腔球囊导管造影相比,改良双腔球囊导管配合阴道超声引导法插管困难率(0.58%)显著降低(P<0.01);与常规双腔球囊导管造影相比,改良后双腔球囊导管配合阴道超声引导显影图像完整清晰,子宫腔显示完整。结论应用改良双腔球囊导管配合阴道超声引导法可提高造影插管成功率,诊断更加准确,降低出现假阳性的可能。Objective To explore the advantages of modified double-lumen balloon catheter combined with vaginal ultrasound guided contrast examination in the diagnosis of malformed uterus and tubal infertility.Methods 1262 cases of infertility patients with hysterosalpingography(HSG)who were in the reproductive department from February 2018 to February 2020 were collected.742 cases were checked with conventional double-lumen balloon catheter angiography,and 520 cases were checked with modified double-lumen balloon catheter combined with vaginal ultrasound guided angiography.The reason and rates of intubation difficulties between the two groups were compared,the angiographic morphology of normal uterus and abnormal uterus were observed,and the advantages of the accuracy of diagnosis of uterine abnormalities and fallopian tube obstruction were analyzed.Results The difficulty rate of intubation with conventional double-lumen balloon catheter angiography was 14.01%.Compared with conventional double-lumen balloon catheter angiography,the intubation difficulty rate(0.58%)of modified double-lumen balloon catheter combined with vaginal ultrasound guidance was significantly reduced(P<0.01).Compared with conventional double-lumen balloon catheter angiography,the improved double-lumen balloon catheter combined with vaginal ultrasound guided imaging was complete and clear,and the uterine cavity was displayed intact.Conclusion Application of modified double-lumen balloon catheter combined with vaginal ultrasound guidance can increase the success rate of contrast intubation,make diagnosis more accurate,and reduce the possibility of false positives.
