摘 要:目的研究Teach-back健康教育模式结合呼吸功能训练对老年胸腔积液行穿刺引流术后患者肺功能康复的影响。方法选取2017年9月至2019年9月间于我院就诊的老年胸腔积液行穿刺引流术后患者135例,按照随机数字表法分为对照组(67例)和研究组(68例)。对照组术后给予呼吸功能训练,研究组在对照组的基础上,术后增加Teach-back健康教育模式。观察干预2周后,两组患者的呼吸功能锻炼依从性、肺功能康复效果、并发症发生率指标方面的改变。结果研究组呼吸功能锻炼依从性显著高于对照组(82.35%vs 71.06%,P<0.05)。干预后研究组肺活量(Vital capacity,VC)、潮气量(Tidal volume,TV)、第1秒用力呼气量(Forced expiratory volume in 1 second,FEV1)及屏气时间(Breath-hold time,BHT)均显著高于对照组(P<0.05)。研究组并发症发生率显著低于对照组(7.35%vs 17.93%,P<0.05)。结论 Teach-back健康教育模式结合呼吸功能训练,应用于老年胸腔积液行穿刺引流术后的患者中,可通过提高患者呼吸功能锻炼依从性来提高其肺功能康复效果、降低并发症的发生,可进行临床推广。Objective To study the effect of teach-back health education mode combined with respiratory function training on pulmonary function rehabilitation of elderly patients with pleural effusion after puncture and drainage.Methods 135 elderly patients with pleural effusion after puncture and drainage were selected from September 2017 to September 2019 in our hospital.They were divided into control group(67 cases)and study group(68 cases)according to the random number table method.The control group received postoperative respiratory function training,and the study group was added by a teach-back health education mode on the basis of the control group.The respiratory exercise compliance,pulmonary function rehabilitation effect and complication rate of the two groups after 2 weeks of intervention were observed and compared.Results The compliance rate of respiratory function exercise in study group was significantly higher than that in control group(82.35%vs 71.06%,P<0.05).After intervention,vital capacity(VC),tidal volume(TV),and forced expiratory capacity(FEV1)in the study group were significantly higher than those in the control group(P<0.05).BHT was significantly longer than the control group(P<0.05).The complication rate of the study group was significantly lower than that of the control group(7.35%VS 17.93%,P<0.05).Conclusion The application of the teach-back health education model combined with respiratory function training in elderly patients with pleural effusion after puncture and drainage can improve the pulmonary function rehabilitation effect and reduce the complication response by improving the patient’s compliance with respiratory function exercise,which can be promoted clinically.
