腹腔镜下骶骨固定术与曼氏手术治疗子宫或阴道脱垂的效果比较

摘　　要：目的比较腹腔镜下骶骨固定术与曼氏手术治疗子宫或阴道脱垂的效果。方法回顾性分析30例因子宫或阴道脱垂接受腹腔镜骶骨固定术的盆腔器官脱垂定量分期(POP-Q)为Ⅱ、Ⅲ期的患者资料(研究组);选取接受曼氏手术的同期30例为对照组。比较两组患者的围术期情况、手术效果及术后6个月生活质量评分。结果研究组手术时间较对照组长,但术中出血量少于对照组(P<0.05)。两组患者术后并发症的发生率差异无统计学意义(P>0.05)。术后随访6个月,研究组患者POP-Q评分中的Aa、Ba、C、Ap、Bp和D点长度均较术前及同期对照组长,PISQ-12、PGI-C评分亦均优于同期对照组(均P<0.05)。结论与曼氏手术比较,腹腔镜下骶骨固定术治疗子宫或阴道脱垂具有术中出血量少、远期预后良好等优势,改善患者性生活质量更为显著。Objective To compare the effect of laparoscopic sacral fixation and Mann's surgery in the treatment of metroptosis or coleoptosis.Methods The clinical data of 30 patients(Study Group)with the POP-Q StageⅡorⅢwho underwent laparoscopic sacral fixation due to metroptosis or coleoptosis were retrospectively analyzed.Another 30 patients with metroptosis or coleoptosis who underwent Mann’s operation during the same period were selected as the Control Group.The two groups were compared in terms of the perioperative situation,operative effect and the quality of life score at postoperative 6 months.Results The Study Group had a longer operation time than the Control Group,but had less amount of intraoperative bleeding than the Control Group(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P<0.05).After the postoperative follow-up for six months,the Study Group had the length of Aa,Ba,C,Ap,Bp and D in the POP-Q score longer than those in the Control Group during same period,and had the PISQ-12 and PGI-C superior to the Control Group during same period(P<0.05 for all).Conclusion Compared with Mann’s operation,laparoscopic sacral fixation has the advantages of less intraoperative bleeding and better long-term prognosis in the treatment of metroptosis or coleoptosis and can more significantly improve the quality of sexual life of patients.
