隧道法腔内剜除术治疗162例高龄高危前列腺增生症临床观察

摘　　要：[背景]探讨隧道法腔内剜除术治疗高龄高危前列腺增生症的临床疗效.[病例报告]回顾性分析2016年1月—2019年6月间收治并行隧道法腔内剜除术的162例高龄高危前列腺增生症患者的临床资料,评价临床疗效.总体手术时间为(60.55±12.6)min,术后住院(5.6±1.3)d.术后3个月随访结果显示,前列腺症状评分改善42.5%,生活质量评分提高62.1%,最大尿流率上升89.7%,剩余尿量减少46.5%.术后15例发生尿失禁,其中压力性尿失禁6例,急迫性尿失禁9例,术后1个月均恢复,无发生严重出血或死亡病例.[讨论]隧道法腔内剜除术治疗高龄高危前列腺增生症具有手术时间短、术后恢复快及并发症少等优点.BACKGROUND To explore the clinical effects of enucleation with tunnel method in the treatment of elderly patients with high risk prostatic hyperplasia. CASE REPORTS Retrospective analysis was performed on the clinical data of 162 elderly patients with high risk prostatic hyperplasia treated by enucleation with tunnel method between January 2016 and June 2019, the clinical efficacy was evaluated. The overall operation time was(60.55±12.6) minutes, the postoperative hospitalization was(5.6±1.3)days. Follow-up results at 3 month after operation showed that the prostate symptom score improved by 42.5%, the quality of life score(QOL)increased by 62.1%, the maximum urine flow rate increased by 89.7%,and the remaining urine volume decreased by 46.5%. 15 patients developed urinary incontinence after operation, including 6 cases of stress urinary incontinence and 9 cases of urge urinary incontinence, all recovered 1 month after operation. There were no cases of severe bleeding or death. DISCUSSION Enucleation with tunnel method in the treatment of elderly high risk prostatic hyperplasia had the advantages of short operation time, quick postoperative recovery and fewer complications.
