腹腔镜阑尾切除术中无钛夹电凝分离阑尾系膜的应用价值

摘　　要：目的探讨腹腔镜阑尾切除术中无钛夹电凝分离阑尾系膜的应用价值。方法选择台山市人民医院胃肠外科2016年1月至2018年12月期间收治的196例阑尾炎患者为研究对象,将患者根据随机数表法分为观察组和对照组,每组98例,两组患者均接受腹腔镜阑尾切除术治疗,且阑尾系膜的处理分别采用无钛夹电凝分离以及应用Hem-o-lok夹的方式。比较两组患者的手术时间、术中出血量、术后肠蠕动恢复时间、住院费用、住院时间以及并发症发生情况。结果两组患者的术中出血量、术后肠蠕动恢复时间及住院时间比较,差异均无统计学意义(P>0.05);观察组患者的手术时间为(37.29±5.18)min,明显短于对照组的(41.14±4.23)min,住院费用为(9002.4±1462.3)元,明显少于对照组的(9610.6±1899.7)元,差异均有统计学意义(P<0.05);观察组患者术后并发症发生率为4.08%,与对照组的3.06%比较差异无统计学意义(P>0.05)。结论腹腔镜阑尾切除术中应用无钛夹电凝分离阑尾系膜和应用Hem-o-lok夹止血均具有确切的效果,且前者时间更短、更加经济。Objective To explore the application value of electrocoagulation without titanium clip in laparoscopic appendectomy.Methods A total of 196 patients with appendicitis,treated in Department of Gastroenterology,Taishan People's Hospital from January 2016 to December 2018,were selected as the study objects.According to random number table method,the patients were divided into the observation group and control group,with 98 patients in each group.Both groups received laparoscopic appendectomy,and the treatment of appendiceal mesangium was performed by electrocoagulation without titanium clip and Hem-o-lok clip,respectively.The perioperative clinical indicators of the two groups were compared,including operation time,intraoperative bleeding volume,postoperative peristalsis recovery time,hospitalization cost,hospitalization time and complications rate.Results There was no significant difference in bleeding volume,recovery time of postoperative peristalsis,and hospitalization time between the two groups(P>0.05);the operation time of the observation group was(37.29±5.18)min,which was significantly shorter than(40.14±4.23)min of the control group,but the hospitalization cost was(9002.4±1462.3)yuan,which was significantly less than(9610.6±1899.7)yuan of the control group(P<0.05).The incidence of postoperative complications in the observation group was 4.08%versus 3.06%in the control group(P>0.05).Conclusion The application of electrocoagulation without titanium clip and Hem-o-lok clip hemostasis in laparoscopic appendectomy has definite effect,and the former is shorter and more economical.
