小儿包皮术中S100可吸收止血材料的临床应用

摘　　要：目的:探讨小儿包皮术中S100可吸收止血材料的临床应用效果。方法:选择中南大学湘雅三医院2018年6月-2019年12月收治的包茎或包皮过长的600例患儿为研究对象。根据双盲法将其分为三组,每组200例。所有患者均给予包皮环切术治疗。C组最内层使用凡士林纱布包扎,A组最内层使用S100吸收性止血绫包扎,B组最内层使用0.9%氯化钠注射液(生理盐水)润湿的S100吸收性止血绫包扎。比较三组患儿术后出血率、伤口换药时疼痛程度、睡眠质量。结果:A、B组伤口换药时疼痛程度评分低于C组,且B组显著低于A组,差异均有统计学意义(P<0.05)。A、B组术后出血率显著低于C组,差异有统计学意义(P<0.05),但A、B组术后出血率比较差异无统计学意义(P>0.05)。三组睡眠质量比较,差异均无统计学意义(P>0.05)。结论:S100可吸收性止血材料能够明显降低小儿包皮手术术后出血率,降低换药时伤口的疼痛程度,且润湿后的S100可吸收性止血材料效果更加显著,值得临床进一步研究及推广。Objective:To explore the clinical application effect of S100 absorbable hemostatic material in prepuce operation of children.Method:From June 2018 to December 2019,600 children with phimosis or redundant prepuce admitted in Xiangya Third Hospital of Central South University were selected as the research objects.They were divided into three groups by the double-blind method,with 200 cases in each group.All patients were treated with circumcision.The innermost layer of group C was wrapped with vaseline gauze,the innermost layer of group A was wrapped with S100 absorbable hemostatic silk,and the innermost layer of group B was wrapped with S100 absorbable hemostatic silk wetted with 0.9%sodium chloride injection(normal saline).The postoperative bleeding rate,pain degree of wound during dressing change and sleep quality were compared among the three groups.Result:The pain degree score of wound during dressing change in group A and group B was lower than that of group C,and the group B was significantly lower than that of group A,the differences were statistically significant(P<0.05).The postoperative bleeding rate of group A and group B was significantly lower than that of group C,the difference was statistically significant(P<0.05),but there was no significant difference between group A and group B(P>0.05).There was no significant difference in sleep quality among the three groups(P>0.05).Conclusion:S100 absorbable hemostatic material can significantly reduce the postoperative bleeding rate of prepuce operation of children and reduce the pain degree of wound during dressing change.Moreover,the effect of S100 absorbable hemostatic material after wetting is more significant,which is worthy of further clinical research and promotion.
