难治性血小板减少性紫癜予以小剂量利妥昔单抗治疗的观察

摘　　要：目的:探讨难治性血小板减少性紫癜予以小剂量利妥昔单抗治疗的临床效果。方法:选取笔者所在医院2015年1月-2020年1月收治的难治性血小板减少性紫癜患者40例,采用随机数字分组法将患者分为两组,对照组给予大剂量丙种球蛋白治疗,研究组给予小剂量利妥昔单抗治疗,对比两组临床治疗总有效率,血小板上升时间、血小板恢复时间,治疗前后外周血淋巴细胞亚群指标。结果:研究组临床治疗总有效率高于对照组,血小板上升时间、血小板恢复时间均早于对照组,差异均有统计学意义(P<0.05);治疗前两组外周血淋巴细胞亚群指标比较差异无统计学意义(P>0.05),治疗后研究组CD8^+、CD4^+、CD4^+/CD8^+均优于对照组,差异均有统计学意义(P<0.05)。结论:难治性血小板减少性紫癜予以小剂量利妥昔单抗治疗,有利于缩短血小板恢复时间,调节T淋巴细胞亚群各指标恢复正常水平,临床疗效显著,值得推广应用。Objective:To investigate the clinical effect of low-dose Rituximab in treatment of refractory thrombocytopenic purpura.Method:A total of 40 patients with refractory thrombocytopenic purpura from January 2015 to January 2020 were selected.The patients were divided into two groups by random digital grouping method.The control group was treated with high-dose Gamma Globulin,and the study group was treated with low-dose Rituximab.The total effective rate of clinical treatment,the time of platelet rise and platelet recovery,and the subsets of peripheral blood lymphocytes before and after treatment were compared between the two groups.Result:The total effective rate of clinical treatment in the study group was higher than that in the control group,the time of platelet rise and platelet recovery were earlier than those in the control group,the differences were statistically significant (P<0.05).Before treatment,there was no statistically significant difference in peripheral blood lymphocyte subsets between the two groups (P>0.05).After treatment,CD8^+,CD4^+,and CD4^+/CD8^+ in the study group were all better than those in the control group,the differences were statistically significant (P<0.05).Conclusion:The treatment of refractory thrombocytopenic purpura with low-dose Rituximab is beneficial to shorten the time of platelet recovery and regulate the T lymphocyte subsets to return to the normal level.It has a significant clinical effect and is worthy of popularization and application.
