辨证施治下蕲蛇酶注射液对急性缺血性中风的疗效观察

摘　　要：目的:对比辨证分型下应用蕲蛇酶注射液对急性缺血性中风的疗效。方法:结合闽南地区特点,选取2014年1月-2016年7月笔者所在医院收治的244例急性缺血性中风患者,按辨证分为风火上扰组85例,静滴蕲蛇酶注射液基础上口服镇肝熄风汤;风痰瘀阻组81例,静滴蕲蛇酶注射液基础上口服半夏白术天麻汤;气虚血瘀组78例,静脉滴注蕲蛇酶注射液基础上口服补阳还五汤。14 d为1个疗程。结果:治疗前三组病情评分比较差异无统计学意义(P>0.05),治疗后三组评分均低于治疗前,且气虚血瘀组评分低于风火上扰组、风痰血瘀组,差异均有统计学意义(P<0.05)。气虚血瘀组治疗总有效率优于风火上扰组、风痰瘀阻组,差异有统计学意义(P<0.05)。结论:辨证分型下应用蕲蛇酶注射液配合中药治疗急性缺血性中风有效,其中益气活血法治疗气虚血瘀型中风具有更好的临床效果。Objective:To compare the therapeutic effect of Agkistrodon enzyme injection on acute ischemic stroke under dialectical classification.Method:Combined with the characteristics of southern Fujian,244 patients with acute ischemic stroke admitted to the author’s hospital from January 2014 to July 2016 were selected,according to differentiation of symptoms and signs,they were divided into wind fire disturbance group (85 cases),Zhengan Xifeng Decoction was taken on the basis of intravenous drip of Agkistrodon Enzyme Injection,Banxia Baizhu Tianma Decoction was taken on the basis of intravenous drip of Agkistrodon enzyme injection in the wind phlegm stasis group (81 cases),and Buyang Huanwu Decoction was taken on the basis of intravenous drip of Agkistrodon Enzyme Injection in the Qi deficiency and blood stasis group (78 cases).14 days was a course of treatment.Result:There was no statistically significant difference in the disease score of the three groups before treatment (P>0.05),and the scores in the three groups after treatment were all lower than those before treatment,and the score in the Qi deficiency and blood stasis group were lower than those in the wind fire disturbance group and the wind phlegm stasis group,the differences were statistically significant (P<0.05).The total effective rate in Qi deficiency and blood stasis group was better than that in wind fire disturbance group and wind phlegm stasis group,and the difference was statistically significant (P<0.05).Conclusion:The treatment of acute ischemic stroke by using Agkistrodon Enzyme Injection combined with traditional Chinese medicine under dialectical classification is effective,and the method of invigorating Qi and activating blood circulation has better clinical effect in treating stroke with Qi deficiency and blood stasis type.
