摘　　要：目的:探讨环磷酰胺联合维生素D治疗狼疮性肾炎(LN)的效果及对肾小球滤过率的影响。方法:选取2017年9月-2019年4月在本院接受治疗的80例LN患者作为研究对象,按照随机数字表法将其分为观察组(n=40)和对照组(n=40)。对照组采用环磷酰胺治疗,观察组在对照组基础上联合维生素D治疗。比较两组治疗前后肾功能相关指标、SLEDAI评分以及临床疗效。结果:治疗后,观察组24 h尿蛋白定量(24 h Upro)和尿素氮(BUN)水平均低于对照组,且估算的肾小球滤过率(eGFR)高于对照组(P<0.05)。治疗后,观察组SLEDAI评分低于对照组(P<0.05)。观察组治疗总有效率为87.5%,高于对照组的67.5%(P<0.05)。结论:环磷酰胺联合维生素D能有效改善LN患者的肾功能,同时降低系统性红斑狼疮疾病活动度,提高临床疗效,具有良好的临床应用价值。Objective:To investigate the effect of Cyclophosphamide combined with vitamin D on lupus nephritis(LN)and its effect on glomerular filtration rate.Method:A total of 80 LN patients treated in our hospital from September 2017 to April 2019 were selected as study subjects.They were divided into observation group(n=40)and control group(n=40)according to the random number table method.The control group was treated with Cyclophosphamide,and the observation group was treated with vitamin D on the basis of the control group.Renal function related indexes,SLEDAI score and clinical efficacy were compared between the two groups before and after treatment.Result:After treatment,24 h urinary protein quantification(24 h Upro)and BUN in the observation group were lower than those in the control group,and the estimated glomerular filtration rate(eGFR)was higher than that of the control group(P<0.05).After treatment,SLEDAI score of the observation group was lower than that of the control group(P<0.05).The total effective rate in the observation group was 87.5%,higher than 67.5%in the control group(P<0.05).Conclusion:Cyclophosphamide combined with vitamin D can effectively improve renal function in patients with LN,reduce the disease activity of systemic lupus erythematosus(SLE),improve clinical efficacy,which has good clinical application value.
