摘 要:目的:探讨自制鸟巢在新生儿振幅整合脑电监护中的应用及对患儿睡眠-觉醒周期的影响。方法:选择2018年6月-2019年6月本院新生儿重症监护室收治的200例新生儿为研究对象,按随机数字表法将其分为对照组和观察组,每组100例。对照组基于基础方案采用传统患儿置暖箱中行振幅整合脑电图(aEEG)监测,观察组基于基础方案及对照组联用自制鸟巢进行aEEG监测。比较两组的aEEG连续性及睡眠-觉醒周期、aEEG监测各项数据评分均值、住院时间。结果:观察组患儿的体温波动小于对照组,住院时间短于对照组,睡眠时间、奶量增加、体重增加均多于对照组,差异均有统计学意义(P<0.05)。观察组患儿aEEG连续性电压高于对照组,不连续高电压和不连续低电压均低于对照组,差异有统计学意义(P<0.05);观察组患儿的睡眠-觉醒周期的成熟率高于对照组,差异有统计学意义(P<0.05)。观察组患儿的Co、Cy、B、LB、T评分均值均高于对照组,差异均有统计学意义(P<0.05)。结论:使用自制鸟巢在振幅整合脑电图监测中让患儿体位固定增加舒适度,使其安静准确的呈现脑皮层数据,值得在各级基层医疗机构新生儿病房推广运用。Objective:To investigate the application of self-made bird’s nest in amplitude-integrated EEG monitoring of newborn and its influence on the sleep-wake cycle of infant.Method:A total of 200 newborns admitted to the neonatal intensive care unit of our hospital from June 2018 to June 2019 were selected as study subjects.They were divided into control group and observation group by random number table method,100 cases in each group.The control group was monitored by amplitude-integrated electroencephalogram(aEEG)in the traditional incubator based on the basic scheme,the observation group was based on the basic scheme and the control group was combined with self-made nest for aEEG monitoring.The aEEG continuity,sleep wake cycle,aEEG monitoring data score,length of stay of the two groups were compared.Result:In the observation group,the fluctuation of body temperature was less than that in the control group,the length of hospital stay was shorter than that in the control group,and the sleep time,milk volume and weight gain were all higher than those in the control group,with statistically significant differences(P<0.05).The aEEG continuous voltage in the observation group was higher than that in the control group,and the discontinuous high voltage and discontinuous low voltage were both lower than those in the control group,with statistically significant difference(P<0.05).The maturity rate of sleep-wake cycle in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).The mean scores of Co,Cy,B,LB and T in the observation group were all higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The use of homemade bird’s nest in amplitude integrated EEG monitoring to fix the position of children to increase comfort,so that the cerebral cortex data can be presented quietly and accurately,which is worthy of promotion and application in neonatal wards of primary medical institutions at all levels.
